PICCO容量性指标在感染性休克液体复苏中的临床应用

目的探讨PICCO容量性指标用于感染性休克液体复苏临床治疗过程中的意义。方法选取在重症监护病房接受治疗的感染性休克患者41例,随机将其分为对照组和实验组。对照组20例给予常规传统体液复苏治疗,实验组21例通过PICCO监护仪监测指导治疗,观察2组患者乳酸水平、中心静脉氧饱和度指数、排尿量、使用呼吸机时间、住重症监护病房时间和病死率等指标。结果当患者复苏后12 h、72 h,2组乳酸水平、中心静脉氧饱和度指数、排尿量比较均存在显著性差异(P均<0.05);2组患者使用呼吸机时间、入住重症监护病房时间和病死率比较也存在显著性差异(P均<0.05)。结论 PICCO监护仪对感染性休克体液复苏有重要的临床指导意义,能实现快速有效的组织灌注治疗目标,纠正感染性休克,避免盲目补液对机体造成的损害,可以正确、快速的提供治疗依据,值得在临床治疗中推广使用。

Clinical application of PICCO volume indexes in fluid resuscitation of septic shock

Objective It is to explore the significance of the clinical application of PICCO volume indexes in the treatment of septic shock with fluid resuscitation.Methods 41 cases of patients with septic shock treated in ICU were collected and randomly divided into control group and experimental group.The control group(n=20) was treated with normal traditional fluid resuscitation,the experimental group(n=21) was treated with the resuscitation under the guides of PICCO monitor's monitoring.The indexes such as lactic acid level,central venous oxygen saturation index,voided volume,treatment time of respirator,length of hospital stay in ICU and mortality were observed in the patients of both groups.Results There were significant differences in the level of lactic acid level,central venous oxygen saturation index,voided volume between the two groups after 12h's and 72's resuscitation(P<0.05).There also were differences in treatment time of respirator,length of hospital stay in ICU and mortality between the two groups(P<0.05).Conclusion PICCO monitor has an important clinical guide significance for the resuscitation of septic shock,which can help to reach the fast and effective treatment target,improve septic shock,avoid the damage of blind fluid infusion,provide fast and right treatment evidences,so it is worth being spread in clinic treatment.
