摘    要:目的 :探讨慢性阻塞性肺疾病 (COPD)中医证型的规律性。方法 :采用LogisticforwardLR回归分析法分析 50 0例COPD患者的肺功能、血常规、生化指标与证型的相关性 ,然后对各指标进行两两比较的t检验。结果 :Logistic回归得出的五条方程显示痰热壅肺型与WBC正相关 ,与红细胞体积分布宽度 (RDW)负相关 ;阳虚水泛型与Hb正相关 ,与MEF75(剩余肺活量的 75 %时的呼气流速 )、PEF(呼气流量峰值 )、MCH(红细胞平均血红蛋白量)负相关 ;脾虚型与MEF2 5,NMO(粒细胞百分含量 )负相关 ;肾虚型与RDW正相关 ,与K+负相关。结论 :NMO ,WBC ,Hb,MCH ,MEF75 ,MEF2 5,K+对COPD具有较好的辨证分型价值

Abstract:Objective: It is to study the regularity of typing and diagnose of chronic obstructive pulmonary disease (COPD). Methods: Five hundred patients with COPD were analyzed. General information, pulmonary function, blood analysis and biochemistry of these patients were determined and the relativity between these indexes above-mentioned and typing was analyzed with Logistic forward regression analysis and liner multiple regression analysis, then these indexes undertaken independent-sample t test. Results: As result of analysis, five equations indicated that phlegm-heat in the lung symptoms positively correlated to white blood cell count (WBC) and negative correlated to red cell distribution width (RDW), retention of fluid caused by yang-deficiency negatively correlated to mean cell hemoglobin (MCH) and positively correlated to expiratory flow-rate when 75 percent forced lung capacity were exceeded (MEF75), peak expiratory flow-rate (PEF) and hemoglobin (HGB), spleen-asthenia negatively correlated to MEF25 and NMO. Conclusion: NMO, WBC, Hb, MCH, MEF75 and MEF25 is valuable for typing of COPD.
