温针灸结合羌防袪痹汤治疗寒湿痹阻型膝关节骨性关节炎临床观察

目的观察温针灸结合羌防袪痹汤治疗寒湿痹阻型膝关节骨性关节炎的临床疗效。方法将2018年11月—2019年11月黑龙江中医药大学附属第一医院骨伤科治疗的60例寒湿痹阻型膝关节骨性关节炎患者按照随机数字法分为治疗组和对照组,每组30例。对照组给予塞来昔布胶囊口服治疗,治疗组在对照组的基础上加以温针灸结合羌防袪痹汤治疗,2组均治疗8周。观察2组患者治疗前后肝肾功能指标及中医症状评分、VAS积分、Lysholm膝关节功能评分的变化,评价2组中医证候疗效。结果2组治疗前后肝功能谷丙转氨酶(ALT)、谷草转氨酶(AST)、碱性磷酸酶(AKP)]、肾功能肌酐(Cr)、尿素氮(BUN)]比较差异均无统计学意义(P均>0.05)。治疗后2组中医症状各项积分及总积分、VAS积分、Lysholm膝关节功能积分均较治疗前明显改善(P均<0.05),且治疗组各评分改善情况明显优于对照组(P均<0.05)。治疗后治疗组的总有效率为90.0%(28/30),明显高于西药组的73.3%(21/30),差异有统计学意义(P<0.05)。结论温针灸结合羌防袪痹汤治疗寒湿痹阻型膝关节骨性关节炎疗效确切,可以明显改善患者临床中医症状,缓解疼痛,改善患者膝关节功能,且安全。

Clinical observation on warming acupuncture and moxibustion with Qiang Fang Qubi Decoction in the treatment of knee osteoarthritis of type of cold-dampness impediment obstruction

Objective It is to observe the clinical curative effect of warming acupuncture and moxibustion with Qiang Fang Qubi Decoction in the treatment of knee osteoarthritis of type of cold-dampness impediment obstruction.Methods Sixty patients with knee osteoarthritis of type of cold-dampness impediment obstruction treated in the Department of Orthopedics and Traumatology of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from November 2018 to November 2019 were divided into treatment group and control group according to the random number method,30 cases in each group.The control group was treated with celecoxib capsules orally,and the treatment group was treated with warm acupuncture and moxibustion combined with Qiang Fang Qubi Decoction based on the control group.Both groups were treated for 8 weeks.The changes of indexes of liver and kidney function,TCM symptom score,VAS score,Lysholm knee joint function score before and after treatment were observed in the two groups,and the efficacy of TCM syndromes was evaluated.Results There was no significant difference in liver functionalanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(AKP),renal function(creatinine(Cr),urea nitrogen(BUN)]between the two groups before and after treatment(all P>0.05).After treatment,the scores of TCM symptoms and total scores,VAS scores,and Lysholm knee joint function scores of the two groups were significantly improved compared with those before treatment(P<0.05),and the improvements of the scores of the treatment group were significantly better than those of the control group(all P<0.05).After treatment,the total effective rate of the treatment group was 90.0%(28/30),which was significantly higher than that 73.3%(21/30)in the control group,the difference was statistically significant(P<0.05).Conclusion Warm acupuncture and moxibustion combined with Qiang Fang Qubi Decoction is safe and effective in the treatment of knee osteoarthritis of type of cold-dampness impediment obstruction,which can significantly improve the clinical symptoms of the patient,relieve pain,and improve the patient’s knee joint function.
