中西医结合治疗糖尿病周围神经病变临床观察

目的观察加味当归四逆汤结合大剂量甲钴胺和胰岛素强化疗法治疗糖尿病周围神经病变的临床效果。方法将105例糖尿病周围神经病变患者随机分为2组,均给予胰岛素强化治疗和大剂量甲钴胺静脉点滴。在此西医综合治疗基础上,当归四逆汤组予当归四逆汤口服,西药对照组予甲钴胺片口服,观察2组患者多伦多临床评分系统评分、神经电生理检查和临床疗效。结果 2组治疗前后及2组间多伦多临床评分系统评分、正中神经、腓神经传导速度和临床疗效均有显著性差异(P均0.05)。结论加味当归四逆汤结合大剂量甲钴胺和胰岛素强化疗法治疗糖尿病周围神经病变效果优于单纯西药综合疗法。

Clinical observation of integrated chinese-western therapy on diabetic peripheral neuropathy

Objective It is to observe the clinical effect of modified decoction of Dangguisini combined with high dose mecobalamin and intensive insulin therapy on diabetic peripheral neuropathy(DPN).Methods 105 patients with DPN were randomly divided into 2 groups which were treated with Dangguisini decoction and mecobalamin tablets respectively on the basis of the same western combined therapy(intensive insulin therapy and intravenous drip of high dose mecobalamin).Scores of Toronto clinical scoring system(TCSS),nerve electrophysiological examination and clinical effect of the two groups were observed.Results There were significant differences between pre-and post-treatment of each group,and also between groups of treatment and control in scores of TCSS,nerve conduction velocities of median and peroneal nerve and clinical efficacy.Conclusion The clinical effect of modified decoction of Dangguisini combined with high dose mecobalamin and intensive insulin therapy on DPN was superior to simple comprehensive treatment with western medicine.
