归脾汤加味防治进展期胃癌化疗所致毒副反应的研究

目的观察归脾汤加味在防治进展期胃癌化疗所致毒副反应的临床效果。方法将68例进展期胃癌患者随机分为2组。治疗组34例,应用化疗同时给予归脾汤加味治疗;对照组34例单纯应用化疗。观察2组治疗前后恶心呕吐、腹泻等胃肠道毒副反应、血液系统毒副反应、体质量、生存质量变化情况。结果治疗组患者Ⅲ~Ⅳ度恶心、呕吐、厌食症状明显少于对照组(P均<0.05),而腹泻症状2组无明显差别(P>0.05);治疗组Ⅲ~Ⅳ度白细胞、中性粒细胞减少症发生率明显少于对照组(P均<0.05),但Ⅲ~Ⅳ度贫血发生率与对照组比较无显著性差异(P>0.05);治疗组体质量下降患者明显少于对照组(P<0.05),生存质量改善者明显多于对照组(P<0.05),平均完成化疗周期数明显多于对照组(P<0.05)。结论归脾汤加味对减轻化疗所致毒副反应,提高化疗耐受性,改善患者生存质量有较好效果。

Study of Decoction for Invigorating the Spleen on chemotherapy induced toxicity in patients with advanced gastric carcinoma

Objective It is to observe the curative effect of Decoction for Invigorating the Spleen(DIS) on chemotherapy-induced toxicity in patients with advanced gastric carcinoma.Methods 68 patients were divided into 2 groups randomly:34 patients in therapeutic group were treated with both DIS and chemotherapy,and 34 patients in control group were treated with chemotherapy only.The gastrointestinal and hematological adverse events,and the changes of quality of life and the cycles of chemotherapy which the patients had completed were observed.Results The incidences of Ⅲ to Ⅳgastrointestinal adverse events including nausea,vomiting,anorexia in therapeutic group were significantly lower than that in control group(P<0.05),but there was no significance difference of diarrhea in both groups(P>0.05).The incidence of Ⅲ to Ⅳ leucopenia,neutropenia in therapeutic group were significantly lower than that in control group(P<0.05),but there was no significance difference of anemia in both groups(P>0.05).Compared with that in control group,the patients with weight loss and decreased KPS scores in therapeutic group were fewer(P<0.05),and the average cycles of chemotherapy which the patients had completed was more(P<0.05).Conclusion DIS is effective to reduce gastric cancer chemotherapy-induced toxicity,strengthen the tolerance for chemotherapy and improve the quality of life.
