泮托拉唑与奥美拉唑预防脑出血并发应激性消化道出血的成本-效果分析

目的比较泮托拉唑与奥美拉唑预防脑出血并发应激性消化道出血的经济效果。方法将270例脑出血患者随机分成3组各90例,泮托拉唑组给予泮托拉唑治疗5~10 d,奥美拉唑组给予奥美拉唑治疗,对照组不使用预防应激性溃疡的药物,均治疗观察1周,比较3组应激性溃疡发生率及患者病死率。结果奥美拉唑组、泮托拉唑组及对照组的应激性溃疡发生率分别为14%,10%和29%,成本-效果比分别为456,669和0。奥美拉唑组、泮托拉唑组与对照组比较均有显著性差异(P均<0.05),且奥美拉唑组成本-效果比显著低于泮托拉唑组(P<0.05)。结论泮托拉唑与奥美拉唑均能为脑出血后抑制胃酸,预防应激性溃疡,泮托拉唑更为经济。

Cost effectiveness analysis of pantoprazole and omeprazole for the prevention of cerebral hemorrhage complicated by upper gastrointestinal bleeding

Objective It is to evaluate the cost-effectiveness of pantoprazole and omeprazole for the prevention of cerebral hemorrhage complicated by upper gastrointestinal bleeding.Methods 270 patients with cerebral hemorrhage were divided into 3 groups with 90 cases in each group.The patients were treated with pantoprazole in pantoprazole group,and with omeprazole in omeprazole group,but did nothing in control group.After one week,the incidence rate of stress ulcer and fatality rate were observed in each group.Results The incidence rate of stress ulcer in pantoprazole group,omeprazole group and control group were 14%,10% and 29% respectively,there were significant differences among 3 groups(P<0.05).The cost-effectiveness rate were 456,669 and 0 respectively,there were significant differences among 3 groups(P<0.05).Conclusion Pantoprazole and omeprazole can effectually prevent cerebral hemorrhage complicated by upper gastrointestinal bleeding,but pantoprazole is more economic.
