深静脉血栓住院患者肺栓塞危险因素相关研究

目的探讨急性期和亚急性期深静脉血栓形成(DVT)住院患者发生肺栓塞(PE)的危险因素。方法对153例深静脉血栓形成患者空腹抽血,进行凝血时间、活化的部分凝血活酶时间、纤维蛋白原、C反应蛋白、血沉、D-二聚体、抗心磷脂抗体等检测,同时进行顺行静脉造影或静脉血管超声检查确诊,并通过Logistic回归分析寻找深静脉血栓形成住院患者发生肺栓塞的危险因素。结果 153例深静脉血栓形成患者中,32例发生PE,发生率为20.9%。PE组活化的部分凝血活酶时间、纤维蛋白原、C反应蛋白、D-二聚体水平均高于非PE组,二者比较有显著性差异(P均0.05);2组凝血时间、血沉、抗心磷脂抗体比较均无显著性差异。经Logistic回归分析示C反应蛋白和D-二聚体是造成DVT住院患者发生PE的独立危险因素。结论 C反应蛋白和D-二聚体可能是造成DVT住院患者发生PE的独立危险因素。

Correlated study on risk factors for pulmonary embolism in hospitalized patients with deep vein thrombosis

Objective It is to approach the risk factors for pulmonary embolism(PE) in hospitalized patients with deep vein thrombosis(DVT).Methods The concentrations of prothrombin time(PT),activated partial thromboplastin time(APTT),fibrinogen,Creactive protein(CRP),erythrocyte sedimentation rate(ESR),D-dimer,anticardiolipin antibody(ACA) in serum from 153 patients with DVT were measured.And ascending venography or lower extremities venous ultrasound examination was performed.The risk factors for PE were determined by logistic regression analysis.Results Of the 153 cases,32 cases had PE(20.9%).The concentrations of APTT,fibrinogen,CRP and D-dimer in the PE group were significantly higher than those in the non-PE group(P<0.05).There were no significant differences in the concentrations of plasma PT,ESR and ACA between the two groups.Logistic regression analysis displayed that CRP and D-dimer were the independent risk factors for the development of PE.Conclusion CRP and D-dimer may be the independent risk factors for PE.
