甲状腺激素水平与原发性高血压及其中医证型的相关性研究

目的探讨血清甲状腺激素水平与原发性高血压中医证型的相关性。方法将170例原发性高血压(EH)患者按中医辨证分为阴虚阳亢型(63例)、肝火亢盛型(21例)、痰浊壅盛型(75例)和阴阳两虚型(11例),另选择同期健康体检者30例作为对照组,检测各组血清甲状腺激素T3、T4、TSH、FT3、FT4水平。结果EH组T3水平显著低于对照组;阴阳两虚组T3水平显著低于其他3个证型组;肝火亢盛组FT3值和痰浊壅盛组TSH值在性别间具有显著性差异;阴虚阳亢组60岁以上患者的T3、T4、TSH和FT3水平明显降低。结论EH患者T3浓度值明显降低,与EH严重程度密切相关,对EH辨证分型具有指导意义;甲状腺激素对于不同性别的同一证型EH患者可能存在不同的身体功能调节机制;甲状腺激素在不同中医证型的EH中随年龄呈现不同的发展变化规律。

Study on the correlation between thyroid hormones levels and TCM types of syndromes of essential hypertension

Objective It is to research the relationship between serum thyroid hormones and TCM types of syndromes of essential hypertension(EH).Methods 170 patients with EH were divided into 4 groups according to the TCM types: 63 cases with Yin deficiency and overwhelming Yang(YDOY),21 with overwhelming liver fire(OLF),75 with accumulation of sputum wetness(ASW) and 11 with deficiency of Yin and Yang(DYY).30 cases of healthy people at the same time were chosen as control group.The levels of thyroid hormones including T3,T4,TSH,FT3 and FT4 were determined respectively in every group.Results T3 levels in EH groups were significantly lower than that of control group.T3 level in DYY group was significantly lower than that of the other three groups.Gender differences of FT3 level in OLF group and TSH level in ASW group were both significantly.T3,T4,TSH and FT3 levels of those older than 60 years in YDOY group were significantly lower than that of the younger ones.Conclusion T3 levels of EH patients were remarkably lower than that of normal ones,which is highly related with degree of seriousness of EH,and it is helpful for making out an EH patient's TCM type.Different mechanisms of thyroid hormones regulating function maybe existed in EH patients with different gender but the same TCM types.The changes of EH patient's thyroid hormones along with ages is not alike in different TCM types.
