黄芪治疗扩张型心肌病研究进展

扩张型心肌病无论是因外感致病还是内伤致病,心气亏虚均是其病理发展的必然结果,在气虚的基础上进而出现的痰浊、瘀血、水饮等病理产物,会导致气阴两虚、血脉瘀阻、心阳虚衰甚至阳气暴脱等危重证候。气虚作为本病的中心环节,贯穿于病变发展过程中的各个阶段。气虚作为本病的中心环节,贯穿于病变发展过程中的各个阶段。黄芪中多种活性成分具有调节免疫功能、正性肌力、抑制心室重构、抗心肌纤维化、利尿、抗血栓、抗心律失常等作用,在治疗扩张型心肌病中,具有举足轻重的作用。近年来,随着更多研究的深入及临床实践的开展,黄芪在扩张型心肌病的治疗中取得了一定的疗效。但在药理作用及病理机制上,仍有许多未明的问题,临床上辨证分型仍不规范,疗效标准差异较大,用药剂量、剂型及相关配伍缺乏大规模的临床研究,加大了评价治疗疗效及安全性的难度。

Dilated cardiomyopathy (DCM) due to either exogenous pathogenic or internal injuries,heart qi deficiency is the inevitable result of the development of its pathology.In addition to qi deficiency,pathological products such as phlegm,blood stasis,excessive fluid,etc.may lead to qi and yin deficiency,blood stasis,heart qi deficiency,even s yang-qi sudden collapse and other critical syndromes.Qi deficiency,as the central part of the disease,runs through all stages of the development of the disease.A variety of active ingredients in Huangqi (Radix Astragali) can regulate the immune function and positive inotropic effect,inhibit left ventricular remodeling,resist myocardial fibrosis,promote diuresis,resits thrombosis and arrhythmia,and play important roles in treating DCM.In recent years,with more in-depth research and clinical practice,Huangqi has achieved some effects in the treatment of dilated cardiomyopathy.But in the pharmacological functions and pathological mechanism,there are still many unknown problems;Clinical syndrome differentiation is still not standardized;Curative effect standard difference is big;There is lack of large-scale study of clinical dosage,dosage form and compatibility,which increase difficulty in evaluation of therapeutic efficacy and safety.
