健脾润肺化痰法治疗慢性阻塞性肺疾病急性加重期痰浊阻肺证临床研究

目的:观察健脾润肺化痰法治疗慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)痰浊阻肺证的临床疗效。方法:选取2012年5月—2015年4月在南京中医药大学第三附属医院急诊及重症医学科就诊的AECOPD患者,按照随机数字表法分为治疗组和对照组,每组30例。两组患者均按照病情需要给予卧床休息、持续低流量吸氧、抗感染、解痉平喘、纠正电解质失衡、营养支持等西医常规对症治疗,治疗组另给予健脾润肺化痰法治疗。结果:对照组有效率73.3%,治疗组有效率93.3%,治疗组优于对照组(P0.05);治疗组治疗后症状评分优于对照组(P0.05或P0.01);治疗组治疗后肺功能、动脉血气分析均优于对照组(P0.05)。结论:健脾润肺化痰法治疗慢性阻塞性肺疾病急性加重期痰浊阻肺证临床疗效显著,能改善患者临床症状及肺功能。

The Clinical Research into Acute Exacerbation of Chronic Obstructive Pulmonary Disease with Phlegm Turbidity Obstructing Lung Syndrome Treated with Method of Invigorating Spleen and Moistening Lung and Resolving Phlegm

Objective:To observe the clinical curative effect of method of invigorating spleen and moistening lung and resolving phlegm on acute exacerbation of chronic obstructive pulmonary disease (AECOPD) with phlegm turbidity obstructing lung syndrome.Methods:Sixty patients with AECOPD who were treated in the third hospital attached to Nanning University of TCM between May 2012 and April 2014 were chosen as the research subjects,and divided into the treatment group and the control group according to random number table method,with 30 cases in each group.The ones in both groups were given bed rest,continuous low flow oxygen,anti infection,relieving spasm and asthma,correcting electrolyte imbalance,nutritional support and other conventional symptomatic treatment of Western medicine,while the ones in the treatment group were added method of invigorating spleen and moistening lung and resolving phlegm based on the treatment of the control group.Results:The effective rate of the control group was 73.3％,while that of the treatment group was 93.3％,and the treatment group was better than the control group (P ＜0.05).After the treatment,the treatment group's symptom score was better than that of the control group (P ＜ 0.05 or P ＜0.01).Its pulmonary function and arterial blood gas analysis were all better than those of the control group (P ＜0.05).Conclusion:Method of invigorating spleen and moistening lung and resolving phlegm has a remarkable clinical curative effect on AECOPD with phlegm turbidity obstructing lung syndrome,can improve the patients' clinical symptoms and pulmonary function.
