健脾补肾、活血祛浊法治疗糖尿病肾病临床疗效观察

目的:观察健脾补肾、活血祛浊法治疗糖尿病肾病的临床效果。方法:选取本院2011年7月—2014年10月门诊和病房收治的DN患者80例,按照随机数字表法分为治疗组和对照组各40例,观察结束后治疗组脱落4例,对照组脱落6例。对照组给予基础治疗,治疗组在对照组治疗的基础上给予中药汤剂口服治疗,两组患者均治疗8周。观察两组患者肾小球滤过率、24h蛋白定量及中医证候积分的变化情况。结果:治疗组治疗后肾小球滤过率、24h尿蛋白定量水平均优于对照组,差异有统计学意义(P0.5);两组患者治疗后ALT、AST水平比较,差异无统计学意义(P0.05);治疗组治疗后中医证候积分优于对照组,差异有统计学意义(P0.5);治疗组有效率58.3%,对照组有效率44.1%,两组比较差异有统计学意义(P0.05)。结论:健脾补肾、活血祛浊法治疗糖尿病肾病可提高有效率,改善肾小球滤过率,降低24h尿蛋白定量。

Observation of the Clinical Effects of the Methods of Strengthening Spleen and Tonifying Kidney,Activating Blood and Eliminating Turbidity on Diabetic Nephropathy

Objective:To observe the clinical effects of the methods of strengthening spleen and tonifying kidney,activating blood and eliminating turbidity on diabetic nephropathy (DN).Methods:Totally 80 patients with DN treated in this hospital between July 2011 and October 2014 were randomized into treatment group and control group,40 cases in each group.Four cases in observation group dropped out and 6 cases in control group dropped out.Control group were given basic treatment while treatment group were added with oral administration of Chinese medicine decoction,8 weeks of treatment in both groups.The glomerular filtration rate,protein quantification of 24 h and scores of TCM symptoms in both groups were observed.Results:The glomerular filtration rate,level of protein quantification of 24 h and TCM symptom score in treatment group were superior to those in control group,with statistical significance (P ＜ 0.05).The differences of the levels of ALT and AST between the two groups had no statistical significance (P ＞0.05).The effective rate was 58.3％ in treatment group and 44.1％ in control group,with statistical significance (P ＜ 0.05).Conclusion:The methods of strengthening spleen and tonifying kidney,activating blood and eliminating turbidity can enhance the effective rate of DN and improve the glomerular filtration rate and reduce protein quantification of 24 h.
