益智汤联合奥拉西坦治疗肾虚血瘀型血管性痴呆临床研究

目的:观察益智汤联合奥拉西坦治疗肾虚血瘀型血管性痴呆的临床疗效。方法:将就诊于本院的92例经中医辨证为肾虚血瘀型血管性痴呆患者随机分为对照组和治疗组,每组各46例。对照组给予奥拉西坦注射液,静脉滴注;治疗组在对照组的基础上加用益智汤。观察两组患者临床疗效,比较两组患者治疗前后症状积分、认知能力积分、日常生活能力积分变化情况,检测两组患者治疗前后血液流变及炎性因子水平。结果:治疗组有效率为91.3%,对照组有效率为71.7%,两组患者临床疗效比较,差异具有统计学意义(P0.05)。两组患者治疗后腰膝酸软、眩晕、耳鸣重听、肢体麻木等证候积分显著低于治疗前,且治疗组低于对照组,差异具有统计学意义(P0.05)。两组患者治疗后认知能力、日常生活能力积分高于治疗前,且治疗组高于对照组,差异具有统计学意义(P0.05)。两组患者治疗后血液流变及炎性因子水平优于治疗前,且治疗组优于对照组,差异具有统计学意义(P0.05)。结论:益智汤联合奥拉西坦治疗肾虚血瘀型血管性痴呆疗效显著,能够缓解患者的临床症状,利于认知能力、日常生活能力的提高,改善血液流变,强化脑部血液灌注,抗炎作用显著。

Clinical Research on Vascular Dementia of Kidney Deficiency and Blood Stasis Syndrome Treated with Intelligence-Benefiting Decoction Combined with Oxiracetam

To observe the clinical effects of Intelligence-Benefiting Decoction combined with Oxiracetam on vascular dementia.Methods:Totally 92 patients with vascular dementia of kidney deficiency and blood stasis syndrome differentiated by Chinese medicine were randomized into control group and treatment group,46 cases in each group.Control group were given intravenous Injection of Oxiracetam while treatment group were added with Intelligence-Benefiting Decoction.The clinical effects,symptom scores,cognitive competence and daily life ability in both groups were observed and compared.The levels of hemorheology and infflammatory factors before and after treatment in both groups were tested.Results:The effective rate was 91.3％ in treatment group and 71.7％ in control group;the difference had statistical significance(P ＜ 0.05).The symptoms scores of sourness and softness of the waist and knees,vertigo,tinnitus and hard of hearing,limb numbness greatly declined in both groups after treatment,and the decline in treatment group was greater than that in control group;the difference had statistical significance(P ＜ 0.05).The scores of cognitive competence and daily life ability got enhanced after treatment,and the enhancement in treatment group was greater than that in control group;the difference had statistical significance (P ＜ 0.05).The levels of hemorrheology and inflammatory factors after treatment in both groups were superior to those before treatment,and treatment group were superior to control group;the difference had statistical significance(P ＜ 0.05).Conclusion:Intelligence-Benefiting Decoction combined with Oxiracetam has significant therapeutic effects on vascular dementia of kidney deficiency and blood stasis syndrome,which can relive the clinical symptoms,benefit the cognitive competence daily life quality,improve the hemorrheology,strengthen the cerebral hemoperfusion and anti-inflammatory action.It is worthy of wide clinical application.
