高血压病动态血压与中医辨证分型的关系

摘    要:对高血压病的肝阳上亢和阴虚阳亢型与 2 4h动态血压的关系进行观察 ,结果 :肝阳上亢型 2 4h收缩压的最大、平均及最小值均略低于阴虚阳亢型 ,但差异无显著性 (P >0 .0 5 ) ,2 4h平均压的最大、平均及最小值显著低于阴虚阳亢型 (P <0 .0 5 ) ,2 4h舒张压的最大、平均及最小值均显著高于阴虚阳亢型 (P <0 .0 5 )。两种证型的血压昼夜变化规律亦有所不同。白昼收缩压肝阳上亢型略低于阴虚阳亢型 ,但差异无显著性 (P >0 .0 5 ) ;白昼的舒张压肝阳上亢型略高于阴虚阳亢型 (P >0 .0 5 )。夜间收缩压肝阳上亢型明显低于阴虚阳亢型 (P <0 .0 5 ) ,而夜间舒张压肝阳上亢型仍高于阴虚阳亢型 (P <0 .0 5 )。虽然两种证型皆呈现昼高夜低的特点 ,但肝阳上亢型昼夜比显著高于阴虚阳亢型 ,可见肝阳上亢型血压昼夜变化大 ,而阴虚阳亢型变化较小 ;肝阳上亢型的收缩压累计值和收缩压负荷低于阴虚阳亢型 ,而舒张压累计值和舒张压负荷则高于后者。阴虚阳亢型的脉压差亦显著高于肝阳上亢型。可见 ,肝阳上亢型以舒张压负荷升高为主 ,而阴虚阳亢型以收缩压负荷升高为主

Abstract:Objective: To observe the relationship between 24-hour dynamic pressure and two types of hypertension, one characterized by hyperactivity of liver yang and the other hyperactive yang due to yin deficiency. Results: The maximum, average, and minimum systolic pressure of the former within 24 hours is slightly lower than that of the latter, but without significant difference ( P >0.05). The maximum, average, and minimum mean pressure of the former within 24 hours is markedly lower than that of the lower ( P <0.05), and that of the diastolic pressure markedly higher than that of the lower ( P <0.05). Differences also lie in the varying pressure during the day and at night. The systolic pressure during the day in the former is slightly lower than that in the latter but without significance ( P >0.05), while the diastolic pressure slightly higher than that in the latter ( P >0.05). The systolic pressure at night in the former is markedly lower than that in the latter ( P <0.05), while the diastolic pressure higher than the latter ( P <0.05). Although both manifest a high volume during the day and low volume at night, the ratio in the former is markedly higher than that in the latter, which prove a bigger variation in the former. The accumulation and load of systolic pressure in the former are lower than those in the latter, while such figures of diastolic pressure higher than those in the latter. The pulse pressure difference in the latter is also markedly higher than that in the former. This suggests the former is characterized by a rise in diastolic load, wile the latter by a rise in the systolic load.
