中西医结合治疗股骨头缺血性坏死疗效观察

采用髓关节滑膜切除、股骨头钻孔减压、同时植入多条血管束恢复血循，辅以髋周松解、头臼成形、软骨修补及髋臼加盖与内收肌切断术，配合中药内服的方法，收治创伤后股骨头缺血性坏死３１例，经平均３年的随访，有效率９３．５％，优良率９０．３％。结果表明多条血管束植入从数量与质量上保证了股骨头的血液供应，在效果上优于单和血管束植入、肌骨瓣、带血管骨瓣等治疗方法。认为联用多种辅助手术和中医中药疗法，较单用多条血管束植入法，更为合理，疗效更高。

TCM-WM combined treatment of ischemic necr osis of femor al head: A therapeutic-observation

Thirty-one cases of post-traumatic ischemic necrosis of femioral head were treated by senovectomy, trephing and decompressing femoral head, implanting many bundles of blood vessels for blood circulation recovery and applying TCD for oral medication and external washing. Periarticular releasing of hip, capitoacetabular arthroplasty,chondral repair,acetabular capping and adductor amputation,when necessary,were used as the auxiliary means. The follow-up of an average of 3 years showed an effective rate of 93. 5% and an excellent-good rate of 90. 3%. The results suggested that multiple-bundle vascular implant could ensure the quantity and quality of the blood supply to the femoral head and,in results, have an advantage over single-bundle vascular implant, muscle-bone flap and vascularised bone flap. It was considered that combined many auxiliary operations and TCM therapies have more rational surgical design and better therapeutic effects.
