腰椎间盘突出物的位移形变与临床

摘    要:对33例腰椎间盘突出症的住院患者进行硬膜外封闭结合手法整复以及骨盆牵引等方法治疗,对治疗前后椎间突出物CT征象的变化进行分析,发现非手术疗法可使是是盘物在CT像上产生缩小、扩大、左右闰移等形变,但位移开变无论大小均可同时获痊愈,好转、无效三种疗效为发性无菌性炎症水肿是非手术疗法有效的关键因素,利用CT对手法整复疗铲另以评价,应具体情况具体分析,非手术疗法的是恢复发病前或建立发病后无症状的病理平衡状

Abstract:Thirty-three inpatients with protrusion of lumbar intervertebral disc (PLID) were treated by extradural blockade combined with manipulative reduction, pelvis traction. etc, and the CT--sign changes of PLID before and after the treatment were analysed. Through analysis, it was found thatthe non--operative therapies could cause the displacement and deformation of PLID CT image, such as reduction, enlargement, left and right shift.However, three results including cured, improved and ineffective could be simultaneously obtained whether the displacement and deformation wereobvious or not. It was believed that the elimination of the secondary aseptic inflammation and edema could be the key to the effective treatment bythe non--operative therapies. In clinical practice, a concrete analysis should be made on CT evaluation of the therapeutic effect of the manipulativereduction according to specific conditions. The aim of the non-operative treatment is to recover the premorbit. or establish the postmorbit, asymptomatic pathological balance or compensation.
