穴位按压联合硝酸甘油含服治疗冠心病急性心绞痛疗效观察

目的观察穴位按压联合硝酸甘油含服治疗冠心病急性心绞痛的临床疗效。方法87例冠心病急性心绞痛患者,按随机数字表法分为观察组44例和对照组43例。两组均予硝酸甘油片常规剂量和用法,并在此基础上,观察组取膻中、内关、郄门行穴位按压。观察两组症状评分、心绞痛缓解时间、心率及血压,并比较两组临床疗效、心电图疗效。结果观察组临床疗效优于对照组,差异有统计学意义(Z=﹣2.126,P<0.05)。观察组心电图疗效与对照组比较,差异有统计学意义(Z=﹣2.156,P<0.05)。治疗后,两组胸痛、胸闷、心悸及气短症状评分较治疗前明显改善,且观察组优于对照组,差异有统计学意义(P<0.05)。治疗后,与对照组比较,观察组心绞痛缓解时间短、心率平稳及收缩压波动小,差异有统计学意义(P<0.05)。结论采用硝酸甘油舌下含服治疗冠心病心绞痛患者时予穴位按压,不仅提高临床疗效、改善心功能,还能迅速缓解症状,且对心率和血压具有双向调节作用。

Therapeutic Observation of Acupoint Pressing Combined with Sublingual Nitroglycerin for Coronary Heart Disease with Acute Angina Pectoris

Objective To observe the clinical efficacy of acupoint pressing plus sublingual Nitroglycerin in treating coronary heart disease(CHD)with acute angina pectoris.Method Eighty-seven CHD patients with acute angina pectoris were divided into an observation group of 44 cases and a control group of 43 cases by the random number table method.The two groups both took Nitroglycerin sublingually at routine dose.In addition,the observation group was treated with acupoint pressing at Danzhong(CV17),Neiguan(PC6)and Ximen(PC4).The symptom scores,duration of angina,heart rate and blood pressure of the two groups were observed.The clinical efficacy and electrocardiogram(ECG)-related efficacy were compared between the two groups.Result The clinical efficacy of the observation group was superior to that of the control group,with statistical significance(Z=﹣2.126,P<0.05).The ECG-related efficacy of the observation group was significantly different from that of the control group(Z=﹣2.156,P<0.05).After treatment,the scores of chest pain,chest tightness,palpitation and shortness of breath were improved significantly in both groups,and were better in the observation group than in the control group,with statistical significance(P<0.05).After treatment,the angina duration was shorter,and the heart rate and systolic blood pressure were more stable in the observation group compared with the control group,and the differences were statistically significant(P<0.05).Conclusion Based on sublingual Nitroglycerin,acupoint pressing can not only enhance the clinical efficacy and improve cardiac function,but also rapidly mitigate the symptoms and bi-directionally regulate the heart rate and blood pressure in the treatment of CHD with acute angina pectoris.
