中西医结合治疗急性胰腺炎疗效观察

目的观察在西医治疗基础上运用大柴胡汤配合芒硝腹部外敷缓解急性胰腺炎（AP）腹痛、腹胀症状的疗效。方法选取2011年1月—2013年12月辽宁中医药大学附属医院急诊科收治的中医辨证为腑实热结证的AP患者62例,采用随机数字表法分为治疗组33例和对照组29例。2组均给予西医常规治疗,治疗组加用大柴胡汤加减,每日1剂,少量多次口服或分3次胃管注入,并予芒硝500 g脐周腹壁外敷,每12 h更换1次。采用视觉模拟法比较2组患者腹痛缓解时间、首次排便时间并记录住院时间。结果治疗组腹痛缓解时间、腹胀缓解时间、首次排便时间及住院时间均较对照组短,差异有统计学意义（P〈0.05）。结论大柴胡汤配合芒硝腹壁外敷治疗具有缓解AP患者腹痛、腹胀症状,恢复肠道功能等疗效。

Traditional Chinese medicine combined with western medicine treatment for acute pancreatitis

Objective To observe the efficacy of Dachaihu Decoction combined with external treatment of mirabilite on abdominal pain and distension in patients with acute pancreatitis（ AP） based on western medicine. Methods Sixty-two AP patients recruited from January of 2011 to December of 2013 in Hospital affiliated to Liaoning University of TCM were divided into a treatment group（ 33 cases） and a control group（ 29 cases） by random number table method. Both groups were treated with regular western medicine. The treatment group was additionally treated with modified Dachaihu Decoction,one decoction for three times per day by oral or stomach tube injection;external treatment of 500 g mirabilite was used on navel area,once 12 h. The VAS was used to compare relieving time of pain,also the first time of defecation and hospitalization days were also recorded. Methods The relieving time of pain,relieving time of distension,the first time of defecation and hospitalization days in the treatment group were all shorter than those in the control group（ P 0. 05）.Conclusion Dachaihu Decoction combined with external treatment of mirabilite on abdomen can relieve the abdominal pain and distension,and restore the intestinal function in acute pancreatitis patients.
