何首乌肝毒性的物质基础、毒性机制与防控策略

摘    要:近年来何首乌肝毒性问题屡有报道,引起了人们的广泛关注。本文就何首乌肝毒性的物质基础、毒性机制与调控策略进行分析探讨。现有文献及作者课题组的研究显示:何首乌肝毒性物质基础主要为蒽醌类化学成分、二苯乙烯苷类化学成分、鞣质类化学成分和外源性污染物,何首乌肝毒性机制可能与机体药物代谢酶缺陷、特异炎性免疫反应、人类白细胞抗原基因多态性等有关。我们的研究进一步证实二苯乙烯苷分解代谢障碍导致其在体内累积可能是何首乌肝毒性的重要物质基础和致毒机制。可通过建立合理质量评价体系、优化炮制工艺和实施辨证论治、个体化用药等进行综合防控。

Abstract:In recent years, the report of hepatotoxicity of Polygonum multiflorum Thunb. have been repeatedly arising. The hepatotoxicity has greatly drawn public attention. This article mainly reviewed the hepatotoxicity, hepatotoxicity material basis, hepatotoxicity mechanism and hepatotoxicity adjustment strategy of Polygonum multiflorum Thunb.. According to the literature and our reseach results of our team, quinines, stilbenes, tannin and exogenous toxic material are main hepatotoxicity composition in Polygonum multiflorum Thunb.. Hepatotoxicity mechanism is related to drug metabolic enzymes, inflammatory specificity immune response, human leukocyte antigen polymorphism. The metabolic disorder of stilbenes, which accumulates in the body, may be the main hepatotoxicity material basis and hepatotoxicity mechanism. The prevention strategy relating to hepatotoxicity of Polygonum multiflorum Thunb. were to establish a rational quality appraisal, to optimize the processing technology, and to implement dialectical treatment and personalized medicine.
