电针“小海”与“下巨虚”穴对十二指肠溃疡大鼠血清肿瘤坏死因子-α及十二指肠高迁移率族蛋白B 1表达的影响

摘    要:目的:观察电针十二指肠溃疡(DU)模型大鼠的小肠经经合穴"小海"、下合穴"下巨虚"对其血清中肿瘤坏死因子-α(TNF-α)及十二指肠组织中高迁移率族蛋白B 1(HMGB 1)表达的影响,初步探讨"合治内府"中"合"穴的相对特异性。方法:SD大鼠随机分为空白组、模型组、小海组、下巨虚组,每组10只。于大鼠右臀部皮下注射10%盐酸半胱胺建立DU模型,小海组电针"小海"穴,下巨虚组电针"下巨虚"穴,每次30min,每天1次,治疗10d。肉眼观察大鼠十二指肠溃疡情况并评分,双抗体夹心酶联免疫吸附法检测大鼠血清TNF-α含量,免疫组织化学法检测大鼠十二指肠组织中HMGB 1表达。结果:造模后大鼠十二指肠溃疡评分较空白组增高(P0.01),小海组、下巨虚组的评分低于模型组(P0.01),且下巨虚组低于小海组(P0.01);模型组的血清TNF-α含量较空白组增高(P0.01),小海组、下巨虚组的血清TNF-α低于模型组(P0.01),且下巨虚组低于小海组(P0.01);模型组十二指肠组织中HMGB 1表达高于空白组(P0.01),下巨虚组十二指肠组织中的HMGB 1低于模型组(P0.05)。结论:1电针小肠经经合穴、下合穴对十二指肠溃疡均可产生一定治疗作用,可能是通过下调血清TNF-α或十二指肠组织HMGB 1的表达来发挥抗炎作用的;且较之"小海"穴而言,"下巨虚"穴存在相对特异性。2本结果部分证实"合治内府"中的"合"主要应指下合穴。

Abstract:Objective To observe the effect of electroacupuncture(EA)stimulation of"Xiaohai"(SI 8)and"Xiajuxu"(ST 39)on serum TNF-αand duodenal high mobility group protein B 1(HMGB 1)levels in rats with duodenal ulcer(DU),so as to analyze its underlying mechanism in improving DU.Methods Forty SD rats were randomly divided into normal control,model,Xiaohai(SI 8)and Xiajuxu(ST 39)groups(n=10in each group).The DU model was established by intramuscular injection of10% Cysteamine hydrochloride(300mg/kg),followed by taking water containing 1% Cysteamine hydrochloride for 48 h.EA(10Hz/50 Hz,1-3mA)was applied to bilateral SI 8and ST 39 for 30min,once a day for 10 days.The DU score(0-5score)was evaluated according to Moraes' method.Serum TNF-αcontent was determined by ELISA and duodenal HMGB 1expression was measured by immunohistochemistry.Results Compared to the normal control group,the DU score,serum TNF-αcontent,and duodenal HMGB 1expression level of the model group were significantly higher(P<0.01).Following EA intervention,the increased DU score,serum TNF-αcontent,and duodenal HMGB 1expression level were considerably down-regulated in the two EA groups in comparison with the model group(P<0.01,P<0.05)except HMGB 1in the SI 8group(P>0.05),and the effect of EA of"Xiajuxu"(ST 39),the lower-He point,was remarkably superior to that of"Xiaohai"(SI 8)in reducing DU score and TNF-αcontent(P<0.01).Conclusion EA stimulation of SI 8and ST 39 can improve duodenal ulceration in DU rats,which may be related to its effect in down-regulating serum TNF-αand duodenal HMGB 1expression.The therapeutic effect of ST 39 is markedly better than SI 8in relieving duodenal ulceration.
