针刀神经定位法治疗腰椎间盘突出症的临床疗效观察

目的:观察神经定位法针刀治疗腰椎间盘突出症(Lumbar Disc Herniation,LDH)的临床疗效。方法:将160例腰椎间盘突出症患者分为治疗组80例、对照组80例。治疗组予针刀治疗,对照组予针灸治疗,疗程均为2周,均配合腰椎牵引治疗。观察两组患者的临床疗效、视觉模拟评分(Visual Analogue Scale,VAS),日本骨科学会评估治疗(Japanese Orthopaedic Association scores,JOA)下腰痛评分这3个指标的变化。结果:治疗组中,治愈60例,显效12例,有效4例,无效4例;对照组中,治愈24例,显效32例,有效16例,无效8例。两组患者临床疗效比较,Z=-4.561,P<0.05。VAS评分:两组数值均较本组治疗前降低,P<0.05;治疗前后的差值比较,治疗组相较对照组降低明显,P<0.05。JOA下腰痛评分:两组均较本组治疗前增高,P<0.05,治疗组较对照组增高明显,治疗前后差值与对照组比较,P<0.05,各项指标差异均有统计学意义。结论:针刀神经定位法治疗腰椎间盘突出症,能调节腰部力学平衡,显著改善腰腿疼痛、下肢麻木症状,适合推广应用。

The clinical effect of nerve positioning method by needle knife on lumbar disc herniation

Objective:To observe the clinical effect of nerve positioning method by needle knife on lumbar disc herniation.Methods:160 patients were divided into the treatment group and control group.The treatment group was given needle-knife,and the control group was given acupuncture treatment 3 times a week,both of the course of treatment was 2 weeks,and all with lumbar traction treatment.Then observe the changes of clinical efficacy,VAS score and JOA lower back pain score.Results:Comparing the clinical efficacy of the two groups,Z=-4.561,P<0.05.VAS score in the two groups were lower than before treatment,P<0.05,the difference between before and after treatment in the treatment group was significantly reduced compared with the control group,P<0.05.JOA low back pain score:Both groups were higher than before treatment,P<0.05.The treatment group increased significantly compared with the control group.Compared with the control group after treatment,P<0.05,and all differences were statistically significant.Conclusion:The needle-knife nerve positioning method treating lumbar disc herniation can adjust the mechanical balance of the waist,significantly improve the pain of the waist and legs,and numbness of the lower extremities.It is suitable for popularization and application.
