自制中药方剂联合PVP术治疗骨质疏松压缩性脊柱骨折临床疗效观察

目的:观察自制中药方剂联合经皮穿刺椎体成形术(Percutaneous Vertebro Plasty,PVP)治疗骨质疏松压缩性脊柱骨折临床疗效,为临床提供依据。方法:选取2017年7月-2018年11月期间我院骨科住院部就诊符合纳入标准的96例骨质疏松压缩性脊柱骨折患者,随机分为治疗组和对照组。对照组予以常规PVP手术治疗,治疗组予以PVP手术治疗后开始规范服用自制中药方剂,出院后随访6个月。比较两组患者入院时及治疗后6个月的视觉模拟评分法(Visual Analogue Scale,VAS)评分、椎体高度恢复程度、Cobb角矫正程度、Oswestry功能障碍指数(Oswestry Disability Index,ODI)评分、骨密度(Bone Mineral Density,BMD)改善及邻近椎体骨折再发生率等指标。结果:治疗组在治疗6个月后患者的VAS评分、ODI评分低于对照组(P<0.05);治疗组患者椎体恢复的高度、Cobb角矫正程度、BMD均高于对照组(P<0.05);治疗组患者的邻近椎体再骨折发生率低于对照组(P<0.05)。结论:自制中药方剂联合PVP术治疗老年骨质疏松压缩性脊柱骨折较常规行PVP手术治疗效果好,可有效的治疗骨质疏松,预防骨折的再发,值得推广。

Observation on treating osteoporotic compressive spinal fracture by TCM medicine plus PVP

Objective:To observe the clinical efficacy of self-made TCM medicine plus PVP on osteoporotic compression spinal fractures.Methods:96 patients were randomly divided into the treatment group and control group.The control group received conventional PVP surgery.After PVP surgery,the treatment group was treated with TCM medicine.The VAS score,the degree of vertebral height recovery,the degree of Cobb angle correction,the ODI score,BMD score and recurrence rate of adjacent vertebral fractures were compared between groups at the time of admission and 6 months after treatment.Results:The VAS score and ODI score in the treatment group were lower than the control group after 6 months(P<0.05).The height of vertebral recovery,Cobb angle correction and BMD in the treatment group were higher than the control group(P<0.05),the incidence of adjacent vertebral fractures in the treatment group was lower than that in the control group(P<0.05).Conclusion:The self-made TCM medicine plus PVP on treating senile osteoporotic compression spinal fracture shows a better effect than conventional PVP surgery,can effectively treat osteoporosis and prevent recurrence of fracture,is worthy of promotion.
