影响针刺治疗面神经麻痹效果的相关因素分析

目的：探讨影响针刺治疗周围性面神经麻痹效果的相关因素。方法：对300例针刺治疗的周围性面神经麻痹患者的临床资料进行回顾性研究，行单因素分析和多因素Logistic分析。结果：①患者年龄越大、针刺介入时机越晚、面神经损伤程度，临床疗效越差，R=0．164、P=0．004，R=0．295、P=0．000，R=0．229、P=0．000；②面神经损伤定位不同，临床疗效不同，单纯性面瘫临床疗效最佳、bell面瘫次之、Hunt面瘫疗效最差，J-T=6．895，P=0．000：③不同中医分型的临床疗效差异有统计学意义，凤寒阻络、风热外袭型疗效较佳，痰瘀阻络和气虚血瘀型疗效较差，J-T=4．752，P=-0．000；④年龄（OR=0．753，95％CI：0．579～0．979）、针刺介入时机（OR=0．556，95％CI：0．420～0．763）、损伤程度（OR=0．589，95％CI-0．388～0．895）和损伤部位（OR=48．409，95％CI：11．527～203．283，OR=6．041，95％CI：2．481～14．707）入选以临床疗效为结果变量的二分类Logistic回归模型，P〈0．05。结论：患者年龄、针刺介入时机、损伤程度和部位是影响针刺治疗周围性面神经麻痹效果的独立因素。

Analysis on the related factors of efficacy in treating facial paralysis with acupuncture therapy

Objective： To investigate the related factors of treating facial paralysis with acupuncture therapy. Methods： The clinical datas of 300 FP patients with acupuncture therapy were applied for retrospective study, and single factor analysis and multiple factor Logistic analysis were done. Results： First, The elder the patient was and the later the treatment was given, the severer the injury of facial nerve remained. Second, different localization of FP would lead to different efficacy. The simple FP had the best efficacy, the Hunt FP had the worst, and the bell FP was the middle. Third, significant differences were detected among the clinical efficacies of different TCM classification. Forth, the age, the acupuncture therapy given time, the injury degree and the pars affecta were selected to be variances in the logistic regression model. Conclusion： The age, acupuncture therapy given time, injury degree and pars affecta were independent factors to the efficacy of facial paralysis by acupuncture therapy, effective in treating, worth a wide clinical application.
