芪麦强心汤对慢性心力衰竭患者窦性心率震荡影响的临床研究

目的:观察慢性心力衰竭患者窦性心率震荡现象(HRT),研究其与心率变异性(HRV)及心功能指标间的相关性,观察芪麦强心汤对慢性心力衰竭HRT的影响。方法:选择慢性心力衰竭患者及正常健康对照者各100例,根据24h动态心电图资料,计算震荡初始(TO)及震荡料率(TS),比较两组之间的差别,并分析TO及TS与LVEF、LVEDD、SDNN及SDANN的相关性,并把心力衰竭患者随机分为常规治疗及芪麦强心汤组,观察治疗半年后HRT的变化。结果:病例组的TO、TS、SDNN、SDANN、LVEF及LVEDD显著小于对照组,病例组的HRT与SDNN、SDANN、LVEF和LVEDD明显相关,治疗半年后芪麦强心汤组的HRT显著好于常规治疗组治疗后的HRT。结论:心力衰竭患者HRT减弱,HRT可能作为评价心力衰竭患者自主神经功能与预测心力衰竭患者碎死的指标,芪麦强心汤明显改善心力衰竭患者的HRT。

Clinical study of impact of the Qimai Qiangxin decoction against HRT of patients with chronic heart failure

Objective:To observe sinus heart rate turbulence(HRT) of the chronic heart failure patients,study the correlation between HRT and heart rate variability(HRV) and cardiac function index,observe the affection of the Qimai Qiangxin decoction for the HRT of chronic heart failure.Methods:100 cases of patients with chronic heart failure and healthy controls,analyzed 24h Holter ECG data to calculate TO and TS,and to compare the difference between the two groups,analysis of correlation between HRT and LVEF,LVEDD,SDNN and SDANN.Patients with heart failure were randomly divided into conventional therapy and the Qimai Qiangxin decoction group,observed the change of HRT after six months treatment.Results:TO,TS,SDNN,SDANN,LVEF and LVEDD of the Qimai Qiangxin decoction group were significantly less than these of the control group,in the Qimai Qiangxin decoction group,HRT significantly associated with SDNN,SDANN,LVEF and LVEDD.After six months treatment,HRT of the Qimai Qiangxin decoction group was significantly better than that of the conventional treatment group.Conclusion:Heart failure with weakened HRT,HRT may be used to evaluate autonomic function and prediction of sudden death in heart failure patients,the Qimai Qiangxin decoction could significant improve HRT of patients with heart failure.
