慢性浅表性胃炎不同程度胃黏膜损伤时舌象的表征

目的：采用舌象分析仪观察441例慢性浅表性胃炎患者舌象表征,探讨慢性胃炎舌象变化规律,为“舌为胃之镜”理论提供客观依据。方法：慢性浅表性胃炎根据胃镜下诊断分为单纯性浅表性胃炎和伴糜烂、伴返流3组;所有患者舌象均采用舌象分析仪照相及自动软件分析。结果：441例慢性浅表性胃炎患者中分布占前2位的舌质为：红舌281例,占63．7％;淡红舌127例,占28．8％;其中淡红舌占28．8％,非淡红舌共占71．2％,总体分布以红舌为主。3组之间红舌分布为CSG伴返流（68．6％）〉CSG（63．3％）〉CSG伴糜烂（63,2％）;绛红舌分布为CSG伴糜烂（3．4％）〉CSG伴返流（2．9％）〉CSG（1,6％）;黯紫舌分布为CSG伴返流（8．6％）〉CSG伴糜烂（5．7％）〉CSG（1．3％）。舌苔分布为前3住为薄白苔占58．7％,白厚苔占14．2％,黄厚苔占10．7％。从苔色来分,白苔总共380例。黄苔61例,以白苔为主;从舌苔厚度来看,薄苔占61．9％;厚苔占31．3％。以厚苔所占比例为多。3组间薄苔分布CSC伴返流（77．1％）〉CSG伴糜烂（63．2％）〉CSC（60．0％）,P〉0．05。结论：（1）441例慢性浅表性胃炎患者3组舌质变化都以红舌比例最高,可以反映慢性浅表性胃炎的舌象变化规律,但与本病胃黏膜损伤的程度无明显相关性。绛红舌、黯紫舌的出现可以提示胃黏膜病变较严重。（2）慢性浅表性胃炎3组舌苔变化都不明显,均以薄白苔为主。

Manifestation in Tongue Pictures of Different Degree Injury of Gastric Mucosa in Chronic Superficial Gastritis

Objective:To explore the theoretical basis of"tongue as the mirror of digestive system diseases" by studying the tongue bodies and coats of 441 chronic superficial gastritis(CSG) patients with a computer programmed tongue analysis instrument.Methods:CSG patients who were diagnosed by gastrointestinal endoscope were divided into three groups: simple chronic gastritis(S-CSG) group,CSG with erosive group,and CSG with back flow group.All patients were taken tongue pictures by a digital camera which was fixed in a box with a stable light source.The records were calculated by computer-programmed tongue diagnosis software.Results:Cases of S-CSG were 319 out of 441 CSG patients,CSG with erosive group were 87of them,and CSG with back flow 35.The ratio of Red tongue ranked first in S-CSG with 63.7%.Slight Red tongue was 28.8%,and non-Slight-Red tongue was 71.2%.The ratio of Red tongue was 68.6% in CSG with back flow,63.3% in CSG,and 63.2% in CSG with erosive respectively;Deep Red tongue was 3.4% in CSG with erosive,2.9% in CSG with back flow,and 1.6% in CSG respectively among three groups.Cruentous tongue was 8.6% in CSG with back flow,5.7% in CSG with erosive 1.3% in CSG respectively among three groups.The distribution of tongue coats in CSG were thin and white(58.7%),white and thick coat(14.2%),and yellow and thick coat(17.89%);as to color,rate of white coat took the proportion of 58.7%,and yellow coat 14.2%;speaking with depth,thin coat took the proportion of 61.9% when thick coat 31.3%.The distribution of thin and white coat among three groups were 77.1% in CSG with back flow,63.2% in CSG with erosive,60.0% in CSG,compared P>0.05.Conclusion:The Red tongue appeared most often in all the 441 patients,it would reflex the characteristics of tongue pictures of chronic superficial gastritis(CSG).But red tongue was of no relation to severity of the disease,while deep red tongue and cruentous tongue might indicate a more severe condition.2.Thin and white coat was the main tongue coat in CSG.
