苈黄导痰汤治疗慢性阻塞性肺疾病急性加重期临床观察

目的:观察苈黄导痰汤治疗慢性阻塞性肺疾病急性加重期的临床疗效并探讨其作用机制。方法:治疗组给予苈黄导痰汤及常规西药,对照组给予常规西药,观察临床疗效、住院时间、主要症状体征缓解时间及血气分析指标等,两组间进行对照。结果:治疗组临床疗效优于对照组(P<0.05);治疗组住院时间(12.5±5.8)天,对照组(14.7±6.3)天(P<0.05);治疗组各主要症状体征缓解时间短于对照组(P<0.05,P<0.01),PaO2、P(A-a)O2的改善优于对照组(P<0.05);治疗组痰浊壅肺、痰热郁肺、外寒内饮三个证型间临床疗效无显著差异(P>0.05)。结论:应用苈黄导痰汤中西医结合治疗COPD急性加重期疗效良好,其作用机理可能与协同抗感染、缓解气道炎症、改善肺通气与弥散功能等有关。

A Clinical Study on Treatment of Chronic Obstructive Pulmonary Disease Exacerbation with Li Huang Dao Tan Decoction

Objective:To assessthe therapeutic efficacyof Li Huang Dao Tan Decoctionon the the acute exacerbation of chronic obstructive pulmonary disease(COPD) and to explore the mechanisms of the effects.Methods: 125patients met a predetermined criteria for an acute exacerbation of COPD were consecutively enrolled and randomly distributed to two groups according to the pathogenetic condition.The 63participants in the treatment groupwere treated with both Li Huang Dao Tan Decoctionandroutine western medicine,the 62participants in the control group were treated withonly routine western medicine.The therapeutic effect,the length of stay,the symptomatic relieftimeand the improvement of artery blood gaswere statisticallyanalyzed andcompared.Results:Members of the treatment group gotbetter therapeutic effect(P<0.05,shorter length of stay(P<0.05),shorter symptomatic relieftime(P<0.05,P<0.01),better improvementof P(a)O_(2)andP_((A-a))O_(2)(P<0.05).Conclusion:The therapeusisof integratedtraditional Chinese and western medicine withLi Huang Dao Tan Decoctionhas favorable therapeutic efficacyon COPD exacerbation.The contributionsof theefficacymighthave been anti-infection,releasing tracheal inflammation,improving pulmonary ventilation and alveolardiffusion,synergistically.
