稳斑汤联合体外反搏治疗冠心病的临床疗效及对患者血清中AngⅡ、VCAM-1的影响

目的观察中药复方(稳斑汤)联合增强型体外反搏治疗冠心病不稳定型心绞痛(痰瘀互结证)的临床疗效及对血清AngⅡ、VCAM-1的影响。方法将120例冠心病不稳定型心绞痛(痰瘀互结证)患者随机分为治疗组与对照组各60例,对照组采用西医基础治疗加稳斑汤治疗,治疗组在对照组治疗方案基础上应用体外反搏治疗,疗程均为4周,疗程前后根据CRF表评定两组受试者心绞痛症状积分、心绞痛症状疗效、中医证候积分、中医证候疗效,采用酶联免疫吸附法测定血清AngⅡ、VCAM-1的含量。结果疗程结束后,与治疗前相比,治疗组和对照组心绞痛症状积分、中医证候积分均有显著降低(P0.05),血清AngⅡ、VCAM-1含量均有显著降低(P0.05),治疗组心绞痛症状积分、中医证候积分均显著低于对照组(P0.05),血清AngⅡ、VCAM-1含量显著低于对照组(P0.05)。治疗组的心绞痛症状疗效、中医证候疗效均显著优于对照组(P0.05)。结论稳斑汤联合体外反搏能有效治疗冠心病不稳定型心绞痛,抑制血浆中AngⅡ、VCAM-1的表达可能是其作用机制之一。

The Clinical Effect of Wenban Decoction combined with External Counterpulsation in the Treatment of Coronary Heart Disease and Its Influence on the Content of Serum AngII

Objective To observe the clinical effect of Chinese herbs compound formula (Wenban decoction) combined with external counterpulsation in the treatment of unstable angina coronary heart disease and its influence on the content of serum AngII and VCAM-1.Method 120 cases of patients with unstable angina coronary heart disease were randomly divided into treatment group and control group, with 60 cases in each group.The control group was treated with basic treatment of Western medicine and Wenban decoction.The treatment group was also given external counterpulsation on the basis of the control group.The course of treatment for both groups was 4 weeks.Before and after the course of treatment, according to the CRF table, the angina symptoms score, angina symptoms curative effect, TCM syndrome score and TCM syndrome curative effect of the two groups were assessed.The enzyme linked immunosorbent assay was used to determine the content of serum AngII and VCAM-1 levels.Results After the course of treatment, comparing with pretreatment, the angina symptom score and TCM syndrome points of the treatment group and the control group were significantly reduced (P<0.05), and the levels of AngII and VCAM-1 in the serum of the treatment group and the control group were significantly decreased (P<0.05).After treatment, the score of angina symptoms score and TCM syndrome points in the treatment group was significantly lower than those of the control group (P<0.05), and the levels of AngII and VCAM-1 in the serum of the treatment group was significantly lower than that of the control group (P<0.05).The effect of angina symptoms and TCM syndrome in the treatment group was significantly better than those of the control group (P<0.05).Conclusion Wenban decoction combined with EECP therapy are more effective in treating unstable angina coronary heart disease, restrain the secretion of serum AngII and VCAM-1 may be one of its mechanisms.
