针刺耳胰胆穴对慢性胆囊炎患者胆囊动力学的影响

目的观察留针时间长短对慢性炎性胆囊运动的影响规律,为确定最佳留针时间、针刺频次提供参考依据。方法将22例慢性胆囊炎患者随机分为30min组和60min组各11例,两组均针刺双耳胰胆穴,分别以中等强度刺激手法捻转行针30s后留针,留针期间每隔10min用同等强度刺激手法捻转行针,每侧穴位30s,两组分别留针30min、60min。分别记录针刺前5min,针刺后10、20、30、40、50、60min时胆囊容积的大小及胆囊收缩率。结果 30min组在针刺后20min时,胆囊容积与针刺前5min比较差异有统计学意义(P<0.05);60min组在针刺20、40、50min时胆囊容积与针刺前5min比较差异均有统计学意义(P<0.05)。两组患者针刺前30min胆囊容积减值幅度变化基本一致,均在针刺后20min时达到收缩的峰值,针刺后30min胆囊容积减值幅度60min组强于30min组,并于留针40min后维持一种稳定的张力状态。两组间不同时间点胆囊容积及胆囊收缩率组间比较差异均无统计学意义(P>0.05)。结论针刺耳廓胰胆穴能够明显促进慢性炎性胆囊的张力性舒缩运动,其最佳留针时间为40min,针刺频次以每天2次为宜。
