糖尿病与胃食管反流病临床流行病学研究

摘    要:目的通过对2型糖尿病患者进行胃食管反流病(GERD)的流行病学调查,探讨其与胃食管反流病发病率的相关性。方法对500例2型糖尿病患者进行反流性疾病问卷调查,其中相关临床指标包括性别、年龄、吸烟、饮酒、BMI、糖尿病病程、空腹血糖、糖尿病并发症等,探讨其与胃食管反流病的相关性。结果糖尿病组GERD发生率(14.2%)较非糖尿病组(7.0%)明显增高(P0.05);糖尿病组中年龄65岁组GERD患病率最高(16.9%),而45岁以下年龄组患病率最低(7.4%);女性人群在糖尿病组中的GERD阳性率(14.8%)明显高于非糖尿病组(5.6%,P0.05);糖尿病病程、糖尿病并发症与糖尿病并发GERD相关(P0.05),吸烟、饮酒、BMI、空腹血糖与GERD无明显相关性(P0.05)。结论糖尿病并发GERD的患病率较高;糖尿病患者中年龄65岁组GERD发病率高于其他年龄组;女性人群在高血糖时发生GERD相关症状的危险性增大;糖尿病病程、糖尿病并发症与糖尿病合并GERD相关。

Abstract:Objective To examine the morbidity of gastroesophageal reflux disease (GERD) and its associated fac- tors in patients with type 2 diabetes mellitus (DM). Methods In 500 DM outpatients, a QUEST inquiry was conduc- ted. Clinical variables (gender, age, smoking, alcohol drinking, BMI, duration of diabetes, fasting blood glucose, dia- betes complications, et al) were surveyed to investigate their association with GERD. Results The incidence of GERD in diabetic groups (14.2%) was significantly higher than that in non-diabetic group (7.0% ,P 〈0.05) ; the incidence of GERD in over 65 years old group was the hishest (16.9%), while that in under 45 years old was the lowest (7.4%). The GERD incidence of the female in the diabetic group ( 14.8% ) was higher than that in the control group (5.6% ,P 〈 O. 05); GERD was correlated with duration of diabetes, complications of diabetes, but not correlated with smoking, alcohol drinking, BMI and fasting blood glucose. Conclusion There is a high prevalence of GERD in diabe- tes mellitus patients. The incidence in over 65 years old group is higher than that in other groups. The incidence in the female is increasing when hyperglycemia occurs. GERD is correlated with duration of diabetes, complications of diabe- tes.
