胃镜对食管裂孔疝的诊断价值分析

目的探讨胃镜对食管裂孔疝的诊断价值，总结该疾病在胃镜下的诊断要点。 方法回顾性分析2015年1月至2016年3月，新疆维吾尔自治区人民医院消化内镜中心确诊滑动型食管裂孔疝的168例患者的临床表现、胃镜特点、并存上消化道疾病，分析其特点。 结果胃镜下诊断滑动型食管裂孔疝188例，经X线确诊168例，符合率为89.4%。胃镜下表现为胃底变浅、His角变钝或消失106例，门齿距齿状线<38 cm 105例，贲门口宽大松弛108例，橘红色胃黏膜疝入食管腔68例，齿状线上移至膈食管裂孔环上>3 cm 81例，并发食管炎78例、Barrett食管29例。 结论胃镜是诊断食管裂孔疝的重要方法，具有确定诊断的价值，同时胃镜检查还能发现其他并存上消化道疾病。

Value of gastroscopy in the diagnosis of esophageal hiatal hernia

ObjectiveTo analyze the value of gastroscope in the diagnosis of esophageal hiatal hernia, and to discuss the diagnostic value of gastroscopy. Methods168 cases of esophageal hiatal hernia were retrospectively analyzed.The clinical manifestations, endoscopic features, and coexisting diseases of upper digestive tract were analyzed. Results188 cases of esophageal hiatal hernia were diagnosed by endoscopy, and the rate of coincidence was about 89.4% with X-ray diagnosis in 168 cases.Endoscopic features of gastric fundus is shallow, blunted His angle or disappeared in 106 cases, incisors from the dentate line was less than 38cm in 105 cases, 108 cases of cardia large relaxation of gastric mucosa, orange hernia into the esophageal cavity in 68 cases, the dentate line to the diaphragmatic hiatal ring, 81 cases of 3cm, 78 cases complicated with esophageal the inflammation of Barrett esophagus in 29 cases. ConclusionGastroscopy is an important method for the diagnosis of esophageal hiatal hernia, and it has the value of definite diagnosis.
