摘    要:目的探讨早期发现脑室-腹腔分流管堵塞3种检测方法的特点和可行性。方法通过多普勒超声、连续CT、MRI,脑室分流管造影3种检查方法来判断脑室分流管的分流情况,对16例脑外伤造成蛛网膜下腔出血、脑室出血、脑疝、外伤性静脉窦回流受阻等原因导致脑积水的患者,行脑室-腹腔分流术,术后2个月至2年对分流管检测。结果9例被判断分流管堵塞的患者,再次手术中与术前检查结果一致。再通术后,5例患者康复水平较前明显提高。结论多普勒超声检查早期即可判断分流管是否通畅。连续CT、MRI检查可进一步帮助判断分流管的分流状况,脑室分流管造影可最后确定堵塞部位,因而能早期诊断和手术。

Abstract:Objective To investigate the character and feasibility of three new methods, which can find ven-triculoperitoneal (VP) shunt's obstruction at early stage. Methods This is a retrospective review of data for 16 patients who received VP shunt from 2002 to 2003. Everyone was examinated by Doppler ultrasound, continuous CT or/and MRT, and VP shunt radiography to decide whether the shunt tube was obstructed. Results 9 patients whose VP shunts were considered obstructed received operation again. The preoperative and intraoperative diagnosis were the same. Conclusion Doppler ultrasound can be used to judge the condition of VP shunt in the early stage, continuous CT or/ and MRT can strengthen the conclusion. VP shunt radiography makes the final diagnosis. The surgery will provide the specific evidence.
