头臂型大动脉炎患者颈动脉转流术后脑血流变化经颅多普勒检查的研究

目的:对头臂型大动脉炎患者颈动脉转流术前、术后的脑血流变化进行研究。方法:对22例头臂型大动脉炎患者颈动脉转流术前、术后分别进行经颅多谱勒检查(TCD),记录颅内血管的峰值血流速度(Vs)、平均血流速度(Vm)、搏动指数(PI)的参数,进行统计学分析。结果:手术侧与非手术侧大脑中动脉(MCA)的Vs、Vm及PI值术前与术后比较差异有统计学意义(P0.01)。基底动脉(BA)的Vs、Vm值术前与术后比较差异无统计学意义(P0.05),PI值术前与术后比较有增高(P0.01)。结论:头臂型大动脉炎患者颈动脉转流手术后颅内主要血管的血流速度及PI值均得到明显增高,脑供血得到很大改善,临床表现明显好转。

A TCD study about the cerebral blood flow change in Takayasu arteritis patients undergoing carotid artery shunting surgery

Objective：To study the cerebral blood flow change in Takayasu arteritis patients who undergoing carotid artery shunting surgery.Methods：TCD detections were carried out separately in 22 Takayasu arteritis patients before and after carotid artery shunting surgery.The data about the Vs,Vm and PI of the cerebral artery were recorded,and the statistic analysis were performed on the data.Results：The Vs、Vm and PI of MCA had significantly increasing in both sugery and non-surgery sides after the carotid artery shunting surgery（P 0.01）.The Vs and Vm of BA had no difference before and after the surgery（P 0.05）,but the PI had significantly increasing（P0.01）.Conclusion：The cerebral blood flow and the clinical situation improved markedly after the carotid artery shunting suegery in Takayasu arteritis patients.
