冠状动脉粥样硬化患者影像表现与冠状动脉循环血浆内皮素-1和一氧化氮水平的关系

摘    要:目的 观察冠状动脉粥样硬化影像表现与冠状动脉循环血浆内皮素 - 1( ET- 1)和一氧化氮 ( NO)水平的关系。方法  5 1例行冠状动脉造影者 ,造影前取血测定冠状静脉窦 ( CS)和冠状动脉开口 ( AO)血浆 ET- 1和 NO水平。冠状动脉腔径狭窄≥ 5 0 %者为严重冠状动脉病变组 ,反之为对照组。分析记录严重冠状动脉病变组病变血管数、狭窄病变数、闭塞病变数和病变范围积分。结果  1.严重冠状动脉病变组 CS血浆 ET- 1水平 ( ET- 1CS)和 CS与AO的差值 ( ET- 1CS- AO)明显高于对照组 ( P<0 .0 5 ) ;严重冠状动脉病变组 ET- 1CS明显高于 AO血浆 ET- 1水平( ET- 1AO) ( P<0 .0 5 )。 2 .严重冠状动脉病变组 NOCS和 NOCS- AO明显低于对照组 ( P<0 .0 5 ) ;严重冠状动脉病变组NOCS明显低于 NOAO( P<0 .0 5 )。 3 .严重冠状动脉病变组 ,ET- 1CS- AO与冠状动脉病变范围积分呈显著正相关 ( P<0 .0 5 ) ,NOCS- AO与冠状动脉病变范围积分呈显著负相关 ( P<0 .0 5 )。结论 冠状动脉粥样硬化影响冠状动脉循环血浆 ET- 1和 NO水平 ,粥样硬化病变范围与冠状动脉循环血浆 ET- 1和 NO水平有相关关系

Abstract:Objective To observe the relation between the manifestations of coronary atherosclerosis angiogram and the plasma levels of endothelin-1 and nitric oxide. Methods In 51 continuous individuals undergoing coronary artery angiography, blood sample for measuring levels of endothelin-1 (ET-1) and nitric oxide (NO) was drawn from coronary sinus (CS) and coronary ostium (AO ) before angiography. If the stenosis in their coronry angiogram was more than 50%, the individuals were recognized as the severe coronary disease group, otherwise, as the placebo group. Using Peter's method, the coronary angiograms of the severe coronary disease group were evaluated for the number of vessels diseased, stenoses, occlusions and the scores of the atherosclerotic extent. Results 1. The levels of plasma ET-1 from CS (ET-1 CS)and the differences between CS and AO(ET-1 CS-AO)in the severe coronary disease group were significantly higher than in the placebo group(P<0.05); and in the severe coronary disease group the levels of ET-1 CS were also more increased than that from AO(ET-1 AO)(P<0.05). 2. The levels of NO CS and NO CS-AO in the severe coronary disease group were significantly less than in the placebo group(P<0.05); and in the severe coronary disease group the levels of NO CS were also more decreased than the levels of NO AO (P<0.05). 3. In severe coronary disease group, the scores of the atherosclerotic extent were positively correlated with the levels of ET-1 CS-AO (P<0.05), but negatively with the levels of NO CS-AO (P<0.05). Conclusion Coronary atherosclerosis may affect the plasma levels of ET-1 and NO in coronary circulation, and the extent of atherosclerosis is correlated with the coronary plasma levels of ET-1 and NO.
