脑钠肽、和肽素和高敏C反应蛋白与急性冠状动脉综合征的相关性

摘    要:目的 探讨急性冠状动脉综合征患者冠状动脉粥样硬化程度与脑钠肽、和肽素和高敏C反应蛋白水平之间的相关性.方法 检测30例急性心肌梗死(急性心肌梗死组)患者,38例不稳定型心绞痛(不稳定型心绞痛组)患者和20例健康对照组血浆脑钠肽、和肽素和血清高敏C反应蛋白水平;比较冠状动脉病变支数、左主干病变、冠状动脉Gensini评分与脑钠肽、和肽素和高敏C反应蛋白水平的相关性.结果 急性心肌梗死组、不稳定型心绞痛组脑钠肽、和肽素和高敏C反应蛋白水平显著高于正常对照组(P<0.01),急性心肌梗死组脑钠肽、和肽素和高敏C反应蛋白水平显著高于不稳定型心绞痛组(P<0.01);脑钠肽、和肽素水平与冠状动脉病变支数、左主干病变、冠状动脉Gensini评分呈正相关(P<0.01),而高敏C反应水平与冠状动脉病变支数、左主干病变、冠状动脉Gensini评分差异无显著性(P>0.05).结论 急性冠状动脉综合症患者脑钠肽、和肽素和高敏C反应蛋白的水平明显升高,和冠状动脉病变程度之间存在相关性.

Abstract:Aim To investigate the correlation of plasma brain natriuretic, C-terminal portion of pro-vasopressin (Copoptin) and serum high sensitivity C-reactive protein in evaluating the prognosis of patients with acute coronary syn-drome. Methods The levels of plasma brain natriuretic, Copeptin and high sensitivity C-reactive protein were meas-ured in 30 patients with acute myocardial infarction (AMI), 38 patients with unstable angina peetoris (UAP}, and 20 con-trols. The numbers of impaired coronary arteries, left main coronary stenosis, Gensini scoring of coronary stenosis and the levels of plasma brain natriuretic, Copeptin and high sensitivity C-reactive protein were compared respectively. Results The levels of plasma brain natriuretic, Copeptin and high sensitivity C-reactive protein in group AMI and group UAP were significantly higher than those in the controls(P<0.01), the levels of them in group AMI were significantly higher than those in group UAP. There were obvious positive correlations in the levels of plasma brain natriuretic and Copeptin with impaired coronary arteries, left main coronary stenosis, and Gensini score;however, there were no correlations in the levels of serum high sensitivity C-reactive protein with them. Conclusions Plasma levels of plasma brain natriuretic, Copep-tin and high sensitivity C-reactive protein correlates with coronary artery stenosis in patients with acute coronary syndrome. They are important clinical parameters to reflect the severity of myocardial necrosis.
