| 摘 要： | 目的:探讨胃黏膜胃动素(MTL)、血管活性肠肽(VIP)的表达与原发性胆汁反流性胃炎之间的关系.方法:标本取自74例原发性胆汁反流性胃炎患者,其中低反流组42例和高反流组32例,24h胃内胆红素监测及电子胃镜检查未见异常者28例为对照组.采用免疫组织化学法检测胃黏膜MTL和VIP表达,并用病理图像分析软件进行分析.结果:高、低反流组较对照组胃黏膜MTL表达明显降低(558.93±223.63,787.09±252.97vs 941.50±275.87,均P<0.05);VIP表达较对照组明显增高(1700.06±486.55.1241.19±359.34 vs 959.00±335.78,均P<0.01).高、低反流组胃黏膜化生性萎缩检出率均较对照组高(50.0%,26.2% vs 7.1%,P<0.01或0.05).化生性萎缩组较浅表性胃炎组胆汁反流程度重(59.3% vs 30.5%,P<0.05).随着胆汁反流程度的加重,幽门螺杆菌检出率下降(64.3%,47.6%vs 25.0%,P<0.01或0.05).结论:胃黏膜MTL及VIP分泌紊乱可能在胆汁反流性胃炎发生中起作用.胆汁反流是胃黏膜损伤的危险因素之一,胆汁反流对幽门螺杆菌感染起抑制作用.
