摘    要:目的以全国范围内非选择性人群为研究对象,探讨连续三次血压测量间的血压变异性及其影响因素。方法对2017年"五月血压测量月"项目中国区域数据(n=77 549)进行分析,所有受试者测量三次诊室血压,以收缩压标准差(SBPSD)、舒张压标准差(DBPSD)、收缩压变异系数(SBPV)、舒张压变异系数(DBPV)作为评估血压变异性的指标。结果研究显示,非所有的第三次血压均较第一次降低,14%的参与者第三次测量的收缩压较第一次增高5 mm Hg或以上。女性SBPV和DBPV高于男性(4.2±3.3)%比(4.1±3.3)%,(4.7±4.0)%比(4.6±4.1)%;P<0.05]。不同年龄、血压级别组SBPSD、DBPSD、SBPV、DBPV差异有统计学意义(均P<0.01)。经多元线性回归分析发现,SBPSD和SBPV与年龄呈负相关,与第一次收缩压读数呈正相关,女性SBPSD和SBPV较男性高(均P<0.01)。结论并非所有血压均随着测量次数的增加而下降。三次血压测量间存在变异性,血压变异性与年龄、性别及血压水平相关。

Abstract:Objective To investigate blood pressure variability among three successive blood pressure measurements in an unselected nationwide population in China. Methods A total of 77 549 participants were included from the May Month Measurement 2017 in China. Blood pressure was measured three times consecutively with a half minute interval. Blood pressure variability was estimated with the standard deviation, coefficient of variation of systolic and diastolic blood pressure. Results Not all participants showed a decreasing trend with increasing number of measurements. In fact, 14% of the participants showed at least 5 mm Hg increase in systolic blood pressure. The coefficient of variation of systolic and diastolic blood pressure in women were higher than in men (4.2±3.3)% vs(4.1±3.3) %,(4.7±4.0)% vs(4.6±4.1)%;P<0.05]. The differences were significant(P<0.01) among different groups of age and blood pressure level. Multiple linear regression analysis showed that the systolic blood pressure variability indexes were inversely associated with age, positively associated with the level of the first systolic blood pressure reading, and SBPSD and SBPV in female were higher than in male(P<0.01). Conclusions Not everybody demonstrates a decreasing trend with increasing number of measurements. Within-visit blood pressure variability varies with age, gender, and blood pressure.
