NT-proBNP评价心房颤动患者舒张功能的临床意义

目的 观察心房颤动患者脑钠肽前体物质（NT-proBNP）的水平与其舒张功能的关系,确定其临床意义.方法 选择77例心房颤动且左室收缩功能正常的患者,给予电复律,在复律前测定其NTproBNP水平,然后在1个月时通过心脏超声评价患者的舒张功能,观察二者的相关性.结果 1个月时心脏超声显示,77例患者中有40例患者心脏舒张功能下降,有37例患者左室舒张功能正常,他们的NT-proBNP值分别是（185.6±96.4）pg/ml和（132.3±82.3）pg/ml,差异有统计学意义.以血清NT-proBNP 151 pg/ml为临界值,判断舒张功能不全的敏感性为92.3%、特异性为89.6%、准确率为93.8%.结论 心房颤动左室收缩功能正常的患者NT-proBNP水平升高,提示患者舒张功能降低.

The clinical significance of NT-proBNP in evaluating the patients diastolic heart function with atrial fibrillation

Objective Now we have no effective methods to evaluate the hearts diastolic function in patients with atrial fibrillation, because the change of NT-proBNP was significantly correlated with heart function, so we observed the relationship between NT-proBNP and hearts diastolic function to determined its clinical significance. Methods 77 atrial fibrillation patients with normal hearts systolic function received electrical conversion. The level of NT-proBNP was measured before received electrical conversion and their diastolic heart function was evaluated one month later. Results One month later, 37 patients hearts diastolic function.were normal, the other 40 patients were lower, the level of NT-preBNP was （132.3±82.3）pg/ml and （185.6±96.4）pg/ml respectively, the difference was statistical significance. Conclusion The NT-proBNP has significant relationship with hearts diastolic function.
