拉米夫定治疗慢性乙型重型肝炎近期疗效观察

目的　观察拉米夫定治疗慢性乙型重型肝炎的近期疗效。方法　选择慢性乙型重型肝炎病人 2 3例 ,随机分成两组 ,拉米夫定组口服拉米夫定 ( 10 0mg/d) ,其它综合治疗措施与对照组相同 ,两组疗程均为 12周 ,观察其临床、生物化学、病毒学指标变化。结果　 ( 1)拉米夫定组TBIL、PTA、Child Pugh计分较对照组下降更显著 (P <0 .0 5 ) ,而ALT、AST、ALB下降和对照组无显著差异。 ( 2 )拉米夫定组HBVDNA阴转率显著高于对照组 ( 88.9%对 3 7.5 % ,P <0 .0 5 )。( 3 )拉米夫定组病死率与对照组无显著差异 ( 18.2 %对 3 3 .3 % ,P >0 .0 5 )。结论　拉米夫定治疗慢性乙型重型肝炎 ,可缓解病情 ,改善肝功能 ,抑制HBV复制 ,但对病死率无显著影响

Observation of short-term therapeutic effect of lamivudine on patients with chronic severe heptitis B

ObjectiveTo observe the short-term effect of lamivudine therapy on chronic severe hepatitis B.Method23 patients with chronic severe hepatitis B were enrolled and randomized into lamivudine group(n=11) and control group(n=12).The lamivudine group was given lamivudine 100mg daily and combined therapy,and the control group was only given the same combined therapy as the lamivudine group,while without being given lamivudine,for 12 weeks.Efficacy was assessed by clinical,biochemical and virologic parameters.Results (1) After 12 week treatment,TBIL,PTA and Child-Pugh score decreasing in lamivudine group were higher than in control group(P<0.05),while ALT,AST and ALB decreasing were no significant difference(P>0.05).(2) HBV DNA negative seroconversion rate in lamivudine group was more higher than in control group(88.9% vs 37.5%,P<0.05) .(3) Fatality rates between lamivudine group and control group were no difference(18.2%vs 33.3%,P>0.05).ConclusionPreliminary data indicate that lamivudine therapy can significantly improve liver function and effectively inhibit HBV replication,but not to effect the fatality rates.
