和肽素和白介素在2型糖尿病患者无症状冠心病中的作用

目的探讨和肽素、白介素（IL-2、IL-4、IL-10）与2型糖尿病无症状冠心病患者的关系。方法选择213例冠状动脉（冠脉）连续造影证实至少一支血管狭窄≥70％的2型糖尿病患者，其中无典型心绞痛史61例（为无症状组），稳定性心绞痛或急性冠脉综合征102例（为症状组）。另选择50例单纯糖尿病患者作为对照组。用放射免疫和ELISA方法测定血清和肽素、白介素和可溶性糖基化终末产物受体（esRAGE）浓度。结果与症状组比较，无症状组和肽素增高，IL-10和esRAGE减低，但三组间IL-2和IL4比较无明显差异。进一步分析显示，症状组中急性冠脉综合征患者较稳定性心绞痛IL-2、IL4、IL-10增高（P〈0．05），和肽素减低。随冠脉病变血管支数增加，IL-10增高、esRAGE减低，但和肽素无显著变化。无症状组患者糖尿病病程较长。结论和肽素及IL-10在2型糖尿病患者无症状冠心病发病中具有重要作用。

Role of copeptin and interleukin in asymptomatic coronary artery disease for patients with type 2 diabetes mellitus

Objective To explore the association of copeptin,interleukin （IL） IL-2,IL-4 and IL- 10 levels with asymptomatic coronary artery disease （CAD） in patients with type 2 diabetes mellitus. Methods Among 163 consecutive patients with CAD（ ≥70% stenosis） and type2 diabetes mellitus,61 were asymptomatic （ Group Ⅰ ）, and 102 had stable angina or acute coronary syndromes （ Group Ⅱ ）. Another 50 patients with pure diabetes were served as a control group. Serum levels of copeptin, IL-2, IL-4, IL-10 and endogenous soluble receptor for glycated end products （esRAGE） were measured using immuno- assay or ILISA methods. Results Copeptin levels were higher but IL-10 levels were lower in Group I than in Group lI. IL-2 and IL-4 levels were similar among the three groups. Further analysis revealed that IL-2 ,IL-4 and IL-10 were significantly higher but copeptin was lower in patients with acute coronary syn- dromes compared with those with stable angina. The number of diseased coronary arteries correlated positively with IL-10, and was inversely related to esRAGE levels. The severity of CAD was not related to copeptin level. The duration of type 2 diabetes was prolonged in patients with asymptomatic coronary artery disease. Conclusion Copeptin and IL-10 may play an important role in the development of asymptomatic CAD in patients with type 2 diabetes.
