左卡尼汀治疗老年2型糖尿病合并慢性心力衰竭患者的临床观察

目的 观察左卡尼汀治疗老年2型糖尿病合并慢性心力衰竭患者的疗效.方法 选择60例老年糖尿病合并慢性心力衰竭患者,随机分为左卡尼汀组及对照组各30例.两组均行常规抗心力衰竭治疗,左卡尼汀组患者在常规治疗基础上加用左卡尼汀2 g静脉泵入,一日两次,共15天,用药前以及开始用药后15天两组均检测左心室射血分数（LVEF）、左心室短径缩短率（LVFS）、心排出量（CO）、血浆氨基末端脑钠素前体（NT-pro-BNP）等指标,并分别对其治疗前后进行比较.结果 左卡尼汀能够增加心力衰竭患者的心肌收缩力及改善心功能,显著降低血NT-pro-BNP水平（P＜0.05）,两组比较差异有统计学意义（P＜0.05）,均未见明显不良反应,且血糖、三酰甘油（TG）、胆固醇（CHO）较治疗前明显下降.结论 左卡尼汀对治疗2型糖尿病合并慢性心力衰竭疗效较好,且无明显不良反应.

Clinical effects of L-carnitine in treatment of type 2 diabetes mellitus with chronic heart faillure in elderly patients

Objective To observe the clinical efficacy of L-camitine in treatment of type 2 diabetes mellitus（T2DM） with chronic heart failure （CHF） in elderly patients.Methods 60 cases elderly DM patients with CHF were randomly divided into L-camitine group（30 cases） and control group（30 cases）.Both groups underwent conventional anti-heart failure treatments.Besides,the L-carnitine group were treated with 2 g of L-carnitine in intravenous pump twice a day in a total amount of 15d.Indicators such as LVDF,LVFS,CO,NT-pro-BNP were detected before treatments and 15 days after medical treatments,and each indicator was compared with the one before the treatments respectively.Results L-camitine increased myocardial contractility in patients with heart failure and improved heart function,significantly reduced blood NT-pro-BNP levels.There were conspicuous differences between the two groups（P ＜ 0.05）,showing no obvious adverse reactions,and GLU,TG,CHO had sharp tendencies of declining compared to those before treatments.Conclusion L-carnitine have a significant effect on the treatments of type 2 diabetes mellitus with chronic heart failure and no obvious adverse reactions.
