非酒精性单纯性脂肪肝患者血清甘丙肽水平及其与炎性脂肪因子相关性

摘    要:目的探讨甘丙肽在非酒精性单纯性脂肪肝(NAFL)人群中的血清水平及其与炎性脂肪因子的相关性。方法2012年2月至2012年4月462名健康体检人群中诊断为NAFL患者65例,以及随机选择同批人群中健康者40名。体检由同-位资深影像学医师利用同-台实时超声显像仪(探头频率3.5MHz)作肝脏检查;血清谷丙转氨酶(ALT)和三酰甘油(TG)用Bayer 1650全自动生物化学分析仪测定;酶联免疫吸附法(ELISA)检测血清甘丙肽、瘦素和单核细胞趋化因子-1(MCP-1)水平;Spearman相关分析检测甘丙肽与炎性脂肪因子的相关性。结果与正常对照组相比,NAFL患者体重指数(BMI)、腹围、血清ALT和TG等均高于健康对照组。NAFL组的血清甘丙肽水平较对照组明显升高(P〈0.05),NAFL组血清瘦素和MCP-1水平均较对照组显著升高(P〈0.05)。相关性分析显示,NAFL组血清甘丙肽与瘦素(r=0.527)、MCP-1(r=0.354)均呈显著正相关(P均〈0.05),而血清甘丙肽与AI,T(r=-0.298)、BMI(r=-0.284)和TG(r=-0.264)均呈负相关(P均〈0.05)。结论NAFL患者血清甘丙肽水平显著增高;血清甘丙肽水平与瘦素、MCP-1等炎性脂肪因子密切相关;甘丙肽可能通过调节相关炎性反应等机制参与非酒精性脂肪性肝病(NAFLD)进展。

Serum level of galanin and its correlation with inflammatory adipokines in patients with nonalcoholic simple fatty liver disease

Abstract:Objective To investigate circulating level of galanin and the relationship between galanin and adipokines in patients with nonalcoholic simple fatty liver (NAFL) disease. Methods Of the 462 subjects who underwent physical examination in People's Hospital of Xinjiang Uygur Autonomus Region, 65 were identified as NAFL patients. These patients and 40 randomly selected healthy adult were included in the study. Hepatic ultrasonographic examinations were performed with a real-time ultrasonic developing-out unit (3.5 MHz) by an experienced sonographer. The alanine arninotransferase (ALT) and triglyceride (TG) were measured with an automatic biochemistry analyzer (Bayer 1650). The serum levels of galanin, leptin, monocyte chemotactic protein-1 (MCP-1) were examed by ELISA. Correlations between galanin and inflammatory adipokines were analyzed by Spearman rank correlation. Results The body mass index (BMI), abdominal circumference, ALT and TG in NAFL patients were higher than these in controls. The serum galanin concentration was significantly higher in NAFL patients than that in controls (P〈0.05) ; Similarly, leptin and MCP 1 were elevated in NAFL patients when compared with controls (P〈0.05) ; Galanin was positively correlated to leptin (r=0. 527) and MCP-1 (r=0. 354) (P〈0.05). However, galanin was negatively correlated with ALT(r = - 0. 298), BMI (r = - 0. 284) and TG (r = - 0. 264). Conclusion The serum galanin was significantly elevated in NAFL patients. The elevated serum galanin was closely correlated with adipokines including lepin and MCP-1. Galanin may play an important role in the pathogenesis of NAFLD through the regulation of inflammatory adipokines.
