脉压指数评价血管硬化的可行性探讨

摘    要:目的 用脉压指数评价血管硬化的可行性 ,比较脉压 (PP)与脉压指数 (PPI)评价血管硬化何者为优。方法 定义脉压指数 (PPI)等于脉压除以收缩压。测量 2 0例老年高血压病患者血压 ,计算每例患者的PP及PPI的变化幅度。把流行病学调查而得 735例老年高血压病患者 ,根据不同的PP及PPI分为四组 ,分析心电图异常与两者的关系 ,同时行Logistic回归分析。结果 PPI的变化幅度要小于PP ,P <0 0 0 0 1;PP与PPI对心电图异常的影响相似 ,但PPI的相对危险性大于PP ,前者为1 86 8,后者为 1 783。结论 用PPI评价血管硬化理论上有依据 ,可从非线形弹性腔理论推得出PPI=(Cs Cd) / (Cs Co) ,并且在一定程度上克服了PP评价血管硬化的缺陷 ,是临床可行的评价方法

Abstract:Objective To study the feasibility of evaluating blood vessel sclerosis using pulse pressure index (PPI), and to find a better method for evaluating vascular sclerosis by comparing PPI with pulse pressure (PP) Methods PPI was defined as PP/SBP PPI=(Cs Cd)/(Cs Co) was derived from the theory of nonlinear elastic cavity This formula reflected the degree of vascular sclerosis theoretically Blood pressure was measured in 20 elderly patients with hypertension, PP and PPI were calculated in each patient According to different PP and PPI, 735 elderly patients with hypertension from epidemiologic survey were classified in to four groups, and the relationship between the values of PP, PPI and abnormality on electrocardiogram(ECG) was analyzed The effect of PP and PPI on ECG abnormality was analyzed by Logistic regression analysis Results The change of PPI was less than that of PP ( P <0 000 1) Both PPI and PP have similar effect on ECG Diastolic pressure decreased with increasing of PPI, not the PP The relative risk of PPI was greater than that of PP(RR=1 868 and 1 783 respectively) Conclusion Degree of vascular sclerosis evaluated by PPI had not only the theoretical basis but also feasibility It can overcome the defect of PP and become a useful parameter in clinical evaluation of vascular sclerosis
