白介素-6基因启动子区域-634C/G多态性与颈动脉粥样硬化的关系

摘    要:目的研究白介素-6基因-634C/G多态性与颈动脉斑块的关系及其与其它危险因素间的交互作用。方法 2005年秋-2006年春,在北京社区整群随机抽样人群中进行流行病学调查,询问病史及生活方式相关危险因素,检测颈动脉内中膜厚度(IMT)、颈动脉斑块、白介素-6基因启动子区域-634C/G位点基因型及相关生化指标。结果共1422例资料完整者进行分析,-634C/C、-634C/G和-634G/G基因型频率分别为46.8%(666)、42.7%(607)和10.5%(149),符合Hardy-Weinberg平衡(p=0.540)。调整年龄和性别后,与-634C/C基因型相比,-634C/G基因型出现颈动脉斑块的危险比值比(OR)为1.12(95%可信限:0.88-1.43,p=0.354),-634G/G基因型为1.49(95%可信限:1.02-2.18,p=0.041)。进一步调整其它危险因素后,结果依然如此。交互作用分析发现,-634C/C+C/G携带者高血压、高胆固醇血症、吸烟与颈动脉斑块的关联强度低于-634G/G携带者,多因素交互作用模型发现,-634C/G、高血压、高胆固醇血症对颈动脉斑块存在显著交互作用(p=0.004)。结论白介素-6基因-634G/G基因型携带者颈动脉粥样硬化危险显著增加,而且这种作用与高血压和高胆固醇血症之间存在交互作用。

Abstract:Objective To explore the association between interleukin-6 (IL-6) -634C/G polymorphisms and early carotid atherosclerosis and the modifi ed effect of risk factors on the association. Methods A community-based survey was carried out in 1,422 participants in rural of Beijing in 2005-2006. Carotid plague was defi ned as carotid Intima Media Thickness (IMT) =1.3mm or a focal raised lesion ≥0.5mm tested by B mode ultrasound. Results The frequencies of -634C/C,C/G, and G/G genotype were 46.8%(666), 42.7%(607), and 10.5%(149) in the study population. After adjusted for age and sex, odds ratio for carotid plaque were 1.12 (95%CI: 0.88-1.43, p=0.354) for -634C/G, and 1.49 (95%CI: 1.02-2.18, p=0.041) for -634G/G as compared with -634C/C in the whole sample. Adjustment for other risk factors did not change this result. More important, the risks of hypertension, hypercholesterolemia, and smoking for carotid plaque were greater in those with -634G/G than those with C/C+C/G. There were signifi cant Interaction of -634C/G polymorphism with hypertension and high cholesterol for developing carotid plaque (p=0.004). Conclusion The interleukin-6 gene polymorphisms of -634C/G is not only signifi cantly associated with carotid plaque, but also signifi cantly modifi ed the risk of hypertension and hypercholesterolaemia on carotid plaque.
