急性冠脉综合征中不同程度冠脉病变患者血浆N末端脑钠肽原检测及其意义

摘    要:目的观察急性冠脉综合征(ACS)患者冠脉严重程度与血浆N末端脑钠肽原(NT-proBNP)的相关性,并探讨其临床意义。方法135例经冠状动脉造影证实的冠心病患者,按冠状动脉造影结果分为:单支病变49例,多支病变86例,15例非ACS患者作为对照组。检测ACS患者12h内及其患者入院时的血浆NT-proBNP浓度,同时行冠脉造影明确冠脉病变性质及程度,比较不同组别NT-proBNP的差异以及冠脉造影结果与NT-proBNP的相关性。结果对照组、单支病变、多支病变组,血浆NT-proBNP浓度分别为89.1±76.3、286.3±378.9、1075.3±1743.0pg/mL,ACS各组血浆NT-proBNP均明显高于对照组(P〈0.05),多支病变组血浆NT-proBNP浓度明显高于单支病变组(P〈0.05)。结论不同ACS冠脉病变程度患者的血浆NT-proBNP浓度存在差异,血浆NT-proBNP浓度与ACS患者冠脉严重程度相关。

Abstract:Objective To explore the relationship between coronary angiography and plasma level of N-terminal pro-brain natriuretic peptide(NT-proBNP) in acute coronary syndrome(ACS) patients. Methods135 patients with ACS confirmed by coronary angiography were enrolled in this study and 15 persons without ACS were selected as control. Lesion phenotypes of ACS included single artery lesion team and multi-arteries lesion team. Plasma levels of NT-proBNP were detected at 12 hours after the appearance of symptoms in ACS and coronary angiographies were taken at the same time. Correlation between NT-proBNP and the result of coronary angiography was studied. Results Plasma levels of NT-proBNP were 89. 1 ± 76. 3,286. 3 ± 378.9,1075.3 ± 1743.0 pg/mL in the patients with control team, single artery lesion team and multi-arteries lesion team, levels of both ACS teams were higher than control team(P 〈 0.05) and level of multi-arteries lesion team was higher than single artery lesion team ( P 〈 0.05 ). Conclusion Plasma NT-proBNP level of different lesion phenotypes of acute coronary syndrome are different, which is associated with severity of coronay artery lesion in ACS patients.
