高血压病胰岛素抵抗与其它代谢的关系与拉西地平干预的影响

目的探讨高血压(EH)患者胰岛素敏感性与其他代谢异常的关系及拉西地平干预作用的影响。方法测定45例EH患者、20例正常人的胰岛素敏感指数(ISI)以及EH患者拉西地平治疗后的变化。结果(1)EH+DSI(ISI↓)组(Ⅰ组，24例)血总胆固醇、甘油三酯、尿酸水平，BMI均比EH+NSI(正常ISI)组(Ⅱ组，21例)显著升高(P<0.05～0.001)，拉西地平治疗后ISI明显升高(P<0.05)；；(2)Ⅱ组的上述指标与对照组无明显差异，治疗后亦无明显改变。结论ISI降低加重了EH患者的其他代谢紊乱，拉西地平可改善ISI的异常。

Relationship between insulin resistance and metabolic abnormality in hypertensiv e patients and effects of lacidipine on them

Objective:To investigate the relationship among insulin sens itivity index(ISI) and plasma lipid、uric acid(Ur),body mass index(BMI) in hypertensive patients(EH) and effects of lacidipine on metabolic abnormality .Methods: ISI was measured in 20 normal subjects (NS) and 45 EH patients before and after 4 weeks of lacidipine treatme nt (4～6mg/d,4 weeks).Results:(1)Total cholesterol,triglycerides(TG),Ur and BMI levels in EH+DSI(ISI↓) group(Ⅰ group,n=24) were significantly higher than those in EH+NSI(normal ISI) group (Ⅱ group,n=21) (P<0.05～0.001),after lacidipine therapy ISI significantly increased(P<0.05),TG significantly decreased(P<0.05);(2)Index as above stated in Ⅱ group was no obvious difference,compared with normal contro ls,aft er therapy also no obvious difference.Conclusion:ISI↓ may aggravate abnormality of plasm lipid and Ur metabolic in patients with EH.Lacidipine can improve ISI and decrease TG level in EH patients with ISI decrease.
