颈椎手术后良性阵发性位置性眩晕的诊治

目的探讨颈椎术后良性阵发性位置性眩晕（BPPV）的诊治。方法回顾分析2012年度颈椎术后后半规管BPPV（PC-BPPV）患者的临床资料。结果4例患者于颈椎术后2—4个月出现单侧PC-BPPV发作，改良侧躺诱发试验提示左侧受累1例、右侧受累3例。给予改良Semont方法治疗经1—3个循环后，患者眩晕症状消失。结论改良侧躺诱发试验和改良Semont方法对颈椎术后BPPV患者安全、有效，颈椎手术不是BPPV诊治的禁忌证。

Diagnosis and treatment of benign paroxysmal positional vertigo after cervical spine surgery

Objective To explore the diagnosis and treatment of benign paroxysmal positional vertigo （BPPV） after cervical spine surgery.. Methods A retrospective analysis of the clinical data of patients with posterior semicircular canal BPPV （PC-BPPV） after cervical spine surgery in 2012 was conducted. Results Four patients presented as unilateral PC-BPPV attack. Modify side-lying test showed that 1 patient involved the left posterior semicircular canal and the other 3 patients involved the right posterior semicircular canals. All patients were free of vertigo symptom after 1 to 3 cycles of modify Semont maneuver. Conclusions It was safe and effective for patients who had cervical spine surgery while suffered from BPPV to undergo underwent modify side-lying test and modify Semont maneuver. Cervical spine surgery was not a contradiction for patients with BPPV to undergo diagnostic and therapeutic procedures.
