摘    要:目的:探讨DNA错配修复基因hMSH6及PTEN表达与40岁以下子宫内膜样腺癌(EEC)的关系。方法:采用免疫组化EnVision法检测hMSH6、PTEN在40例40岁以下EEC组织的表达,并选取40岁以下子宫内膜单纯性增生13例,子宫内膜复杂性增生12例,子宫内膜不典型增生11例为对照。结果:hMSH6、PTEN在子宫内膜单纯性增生、复杂性增生、不典型增生、EEC的表达均呈递减趋势(P=0.002,P=0.005);EEC hM-SH6、PTEN表达率虽低于子宫内膜不典型增生,但差异无统计学意义。hMSH6表达与手术病理分期和子宫肌层浸润有关(P=0.045,P=0.038);PTEN蛋白表达与组织分级有关(P=0.01)。hMSH6与PTEN蛋白在子宫内膜组织的表达呈正相关(rs=0.96,P0.05)。hMSH6、PTEN均表达阳性的EEC组织分化好于均表达阴性者,病理分期早。结论:hMSH6、PTEN基因功能缺失与EEC的早期发生有关,40岁以下EEC hMSH6与PTEN蛋白表达缺失者有预后不良的倾向。

Expressions of DNA mismatch repair gene hMSH6 and PTEN protein in endometrioid endometrial adenocarcinoma under 40 of age
Abstract:Objective:To study the relationship between the expressions of DNA mismatch repair gene hMSH6,PTEN protein and endometrioid endometrial adenocarcinoma(EEC) under 40 of age.Methods:Immunohistochemical EnVision method was used to detect the expressions of hMSH6 and PTEN in 40 cases of EEC,13 cases of simple hyperplasia of endometrium(SH),12 cases of complexity hyperplasia of endometrium(CH),11 cases of atypical hyperplasia of endometrium(AH) under 40 of age.Results:The positive expression rates of hMSH6,PTEN protein were gradually reduced in SH,CH,AH and EEC(P=0.002,P=0.005).Though the positive expression rates of hMSH6,PTEN in EEC were lower than those in AH,the difference had no statistical significance.The positive expression of hMSH6 was correlated with surgical pathologic stage and invasion to myometrium(P=0.045,P=0.038).The positive expression of PTEN was correlated with histological grade(P=0.01).There was positive correlation between the expressions of hMSH6 and PTEN(rs = 0.96,P< 0.05).EEC with both hMSH6 and PTEN positive expressions were likely to be in lower grade and earlier stage than EEC with both negative expression.Conclusion:Loss of hMSH6 and PTEN function maybe related to the early development of EEC.EEC under 40 of age with both hMSH6 and PTEN negative expression are likely to have poor prognoses.
