摘    要:目的探讨介入治疗在重度产后出血中的应用价值。方法三家医院15年间重度产后出血保守治疗无效行次全子宫切除或全子宫切除术47例为手术组,行介入治疗24例为介入组。结果介入组手术时间55.5min,明显短于手术组121.4min(P<0.05);介入组栓塞后止血时间为(5.9±2.6)min;手术组1例行次全子宫切除后复出血转用介入治疗成功止血,余均止血成功。结论介入治疗应用于重度产后出血具有止血快、疗效确切、手术时间短、并发症少的优点。

Abstract:Objective To investigate the value of interventional radiology in severe postpartum hemorrhage.Methods 24 cases of postpartum hemorrhage were performed arteries embolization ( Interventional group ), and 47cases of postpartum hemorrhage who were performed hysterectomy/subtotal hysterectomy in 3 hospitals during 15 years were reviewed as control group(operation group). Results Mean time of procedure in interventional group was 55.5min which was significantly shorter than that of operation group (121.4 min, p<0.05). Time for bleeding stop after embolization was (5.9±2.6) min. Recurrent hemorrhage was observed in one case of operation group who underwent subtotal hysterectomy, and we had successfuly stopped bleeding by interventional technique. Conclusions With very short procedure time and rare complications, interventional radiology is proved to be very effective in the treatment of severe postpartum hemorrhage.
