新生儿窒息时微循环变化的临床意义

摘    要:目的了解新生儿窒息时微循环变化,探讨其临床意义。方法应用耳廓微循环检测技术,对18例轻度窒息新生儿,15例重度窒息新生儿及19例正常新生儿在出生后24小时、48小时和72小时分别进行微循环监测。结果窒息新生儿耳廓微循环中红细胞聚集、白细胞翻滚、附壁等现象明显多于正常新生儿,毛细血管数增多、血流速度减慢,在重度窒息儿尤为明显。结论新生儿窒息时耳廓微循环发生明显变化,重度窒息更为严重且较临床症状消失晚。故对出生时有重度窒息,迅速复苏成功的新生儿,仍应及时给予必要的治疗。

Abstract:Objective To investigate the microcirculation change in asphyxial neonates and toanalyze the clinical significance. Methods By using auricular microcirculation examining technique, the auricular microcirculation in 18 mild asphyxial neonates, 15 severe asphyxial neonates and19 normal neonates were observed at 24 h, 48 h and 72 h after birth separately. Results Microcirculation changed significantly in asphyxial neonates, especia1ly in severe ones. The RBC aggregationrate, neutrophil rolling and adhesion rates, capillary numbers in asphyxial neonates were markedlyhigher and the blood flow velocity in venule were markedly slower than those in normal neonates.Conclusions There is a dramatic change of microcirculation in asphyxial neonates, especially in severe ones. The changes disappear much slower than clinical signs. So even the neonates with asphyxia who are successfully resuscitated and recovered clinically should also be given an early treatment.
