来曲唑应用于冻融胚胎移植内膜准备的研究

摘    要:目的:探索来曲唑(LE)联合hMG促排卵方案应用于冻融胚胎移植(FET)内膜准备的临床效果与预后因素。方法:月经第3日起口服LE连续3~5d,第10日起超声监测,若优势卵泡<14mm,则注射hMG150IU,隔日诱发排卵后92~152h解冻第3~5日胚胎。结果:共纳入119名患者,完成124个FET周期,临床妊娠率为42.7%(53/124),种植率为22.5%(70/311)。妊娠组的年龄显著小于非妊娠组(P<0.05);黄体支持组临床妊娠率为50.6%(45/89),显著高于无黄体支持组的22.9%(8/35)(P<0.01)。结论:在选择性的患者群体中,FET周期中用LE联合hMG准备内膜方法简单可行,年龄、是否采用黄体支持是影响预后的重要因素。

Abstract:Objective:To explore the clinical outcome and prognosis factors in ovulation induction protocol with letrozole(LE) combined hMG.Methods:Oral letrozole 5 mg×3-5 d since the 3rd day of menstrual cycle.hMG was given with 150 IU every other day if the diameter of follicule was 14 mm.All of the patients induced ovulate and day 3-5 embryos were thawed 92-152 h later.Results:Totally 119 patients enrolled and 124 FET cycles were performed,the clinical pregnancy rate was 42.7%(53/124),the implantation rate was 22.5%(70/311).Age and infertile years in pregnant group were significantly lower than those in non-pregnant group(P〈0.05).The clinical pregnancy rate in luteal support group 50.6%(45/89)] was significantly higher than that in non-luteal support group 22.9%(8/35)](P〈0.01).Conclusion:In selected patients group,the protocol of letrozole combined with hMG and induced ovulation in FET cycle is simple and feasibility,satisfactory clinical outcome can be obtained.The important factors of prognosis are age and luteal support.
