摘    要:给付方式是影响长期照护保险制度实施效果的重要因素。作者通过分析现金、服务和混合给付等三种给付模式的实施过程,揭示了三种给付模式的特点,提出了"政府"、"经办机构"、"评估机构"、"护理机构"以及"失能者及其家庭"五个主体对于给付模式选择的影响机制。在此基础上,针对主要的OECD国家以及我国不同的长期护理保险试点地区,分析了这些国家和地区给付模式的演变过程、设计初衷以及优、劣势。最后提出了未来我国长期护理保险制度中给付模式的设计思路。

Abstract:The payment method is an important determinant for the implication outcome of long-term care insurance(short as LTCI)and a factor that affects the effectiveness of LTCI System.By analyzing the implementation process of three types of payment including cash,services,and mixed payments,this study reveals the characteristics of these three types of payment modes,and highlights the effects of five subjects evolving in the payment process on the selection of payment system“government office”,“insurance agency”,“disability assessment organization”,“care service institution”,and“beneficiaries and their families”,respectively.Based on this,the design and evolution of payment systems in major OECD countries and different pilot areas of LTCI in China,as well as their advantages and disadvantages were systematically and relatively analyzed.Finally,the strategic principle is recommended for the improvement of the LTCI payment system in China.
